The San Bernardino County WIC program is conducting a survey to see how well we meet the needs of our clients.  Your input is very important to help our staff improve services to you.  However, your participation is entirely voluntary and whether or not you participate will not affect your relationship with WIC.  Your answers will be kept strictly confidential, and no one in the clinic will ever know how you answered the questions.

This survey should take you about 15 minutes or less to complete.
