  1. Coroner Case #701303847--- On 05/20/2013, at 8:20 PM, a motorcycle driven by Devin E. Bossiere, a 21 year old resident of Alta Loma, was traveling northbound on Vineyard Ave., when he struck a Toyota Sienna that had turned southbound onto Vineyard Ave. from Hemlock St., in Upland. Bossiere was transported by ambulance to San Antonio Community Hospital emergency room where he was pronounced dead of his injuries at 9:25 PM. An autopsy will be conducted to determine the cause of death. The San Bernardino County Sheriff Department is investigating the incident. (05202013 CN 0239)

  2. Coroner case # 701303830 – On 05-20-13, at about 9:30 AM, 49 year-old Raul Elizarraras of Montclair ,was found deceased with upper body trauma in his residence in the 4300 block of Alamitos Avenue in Montclair.   3. Coroner Case #701303816--On 05/19/2013 at 2:41 PM the California Highway Patrol received a 911 call of a traffic collision on Lytle Creek Road, north of Glen Helen Parkway, in the county area of Fontana. Officers arrived on scene to find that two vehicles had collided head on. The driver of one of those vehicles, Martina Johnson, a 72 year-old female resident of Fontana, was transported by paramedics to Kaiser Fontana where she was pronounced dead at 3:50 PM as a result of her injuries . The California Highway Patrol is investigating the traffic collision. (05202013 CN 0430)

  4. Coroner case 701303800: On Saturday, May 18, 2013, at about 4:44 PM, Clarence Jerome Baines, a 33 year old resident of Highland, was found dead in an apartment located in the 1400 block of Date St., in San Bernardino. The death is under investigation by San Bernardino Police Department Homicide Detail. Anyone with information is urged to call 909-384-5742. (CN 05192013 1839)

  5. Coroner Case # 701303775--- On 05/17/2013 at 8:15 PM, Barstow Police Department officers responded to a 911 call for a man who was pinned underneath a pickup truck after being run over in the 2200 block of "P" Street, in Barstow. Officers arrived on scene and found Thomas C. Sinner, an 82 year old resident of Barstow with upper body trauma. Sinner was transported to Barstow Community Hospital where he was pronounced dead of his injuries at 9:16 hours. An autopsy will be conducted to determine the cause of death. Barstow Police Department is investigating the incident. [05192013 0630 EM]

  6. Coroner Case #701303748--On 05/16/2013 at 3:20 pm the California Highway Patrol received a 911 call of a traffic collision on westbound Interstate 40 just west of mile marker 44.5 in the area of Barstow. Officers arrived on scene to find a 2007 Lexus RX350 had run off the highway and rolled over. The right front passenger of the vehicle, Marlene Donahue, age 81 of Murphys, CA was transported via Mercy Air to Loma Linda University Medical center where she was pronounced dead as a result of her injuries at 5:22 pm.   7. Coroner case 701303709--On 5/15/2013 at 12:14 AM Jared Michael Bennett, a 26 year old resident of Highland, was the driver and solo occupant of a his white 1993 Honda Civic traveling eastbound on State Route 210 east of the I-215 interchange at a high rate of speed. For an unknown reason the Honda left the roadway and struck the rear of a parked semi tractor trailer parked on the south shoulder. The California Highway Patrol and San Bernardino City Fire responded and Bennett was pronounced dead at the scene. The CHP is investigating this collision. [05152013 1210 SY]

  8. Coroner Case #701303710 - On 05/14/2013, at 11:28 P.M., the Ontario Police Department received a shots fired call with a man down in the alley to the rear of the 100 block W. Park St., in Ontario. When they arrived they found a 27 year old Hispanic male with trauma to the upper torso. The male was transported to Kaiser Foundation Hospital where he later succumbed to his injuries and was pronounced dead. The Ontario Police Department Homicide Unit is investigating. When the identity is determined and next of kin notified, the name will be released to the public. (05152013 0532 CN)

  9. Coroner case # 701303644 -- On 05-12-13, at 10:54 AM, CHP responded to a single car accident northbound Interstate 15, 2 miles south of Nipton Road in rural Mountain Pass. The driver of the vehicle left the roadway, for unknown reasons, which caused the vehicle to roll over and a passenger to be ejected. The passenger was pronounced dead on-scene. The name of the passenger will be released when family is notified. CHP is investigating the accident. [051213 1630 TC]

  10. Coroner case # 701303636 – On 05-12-13, at 3:49 AM, CHP responded to a traffic fatality on I-15, 3/4 miles N/O Oak Hills Rd, in Hesperia, reference several vehicles that had collided with one another. The investigation revealed that a Ford Mustang rear-ended a Cadillac Escalade. The Mustang was then rear-ended by a Chevy Malibu followed by a Chevy pick-up. The driver of the Mustang, a 30 year-old Apple Valley resident, was pronounced dead on-scene at 3:58 AM. The decedent’s name will be release when family is notified.   11. Coroner case #701303616: On Saturday, May 11, 2013 at 1:09 AM, San Bernardino County Sheriff Department deputies responded to a 911 call for a person stabbed in the 19200 block of 2nd Street, in Oro Grande. Deputies arrived on scene and found a male resident of Oro Grande, with trauma to the upper body. The male was flown by air ambulance to Arrowhead Regional Medical Center where he died of his injuries at 2:41 AM. An autopsy will be conducted to determine the cause of death.   12. Coroner case # 701303614: On Friday, May 10, 2013, at 11:53 PM, San Bernardino Police Department officers responded to a 911 call for shots fired in the 1600 block of Mountain View Ave, in San Bernardino. Officers arrived on scene and found Damon Dion Todd Jr., a 22 year old resident of San Bernardino with trauma to the upper body. Todd was confirmed dead of his injuries at 12:15 AM. An autopsy will be conducted to determine the cause of death. The San Bernardino Police Department Homicide Detail is investigating the incident. [05112013 0917 GB]

  13. Coroner Case #701303588: On 05/10/2013 at 8:48 AM, a 911 call was received reporting a collision on westbound Interstate 40 near mile marker 38. The California Highway Patrol and Desert Ambulance responded to the scene to find a 2000 Jeep Wrangler had struck a semi tractor with 2 belly dump trailers in the rear of the second trailer. The driver of the Jeep, Daniel Ray Douthit, age: 60 from Laughlin, NV, was pronounced dead at the scene. The driver of the semi tractor had just changed lanes in preparation to turn through the median and had slowed to approximately 5 miles per hour when the collision occurred. The California Highway Patrol Major Accident Investigation Team is investigating the incident. [051113 0505 EM]

  14. Coroner case #701303609—On 5-10-2013 at about 9:45 PM, Roger Aguirre Ortiz, age: 21, from Riverside was driving a 2007 Chevy Impala N/B on Archibald Avenue north of Riverside Drive in Ontario when his vehicle left the left road edge and collided with a tree. An Ontario Police Officer on patrol came upon the accident just after it happened. (Note: There were no witnesses to the collision.) Paramedics responded and transported him to Kaiser Hospital in Fontana where he was pronounced dead at 10:17 PM. An autopsy will be performed to determine the cause of death.   15. Coroner case 701303566-- On Thursday, May 9, 2013, at about 11:41 AM, a report was received of a shooting in the parking lot of the Town Center Mall, located at 57725 Twenty-Nine Palms Highway in Yucca Valley. San Bernardino County Sheriff and Fire personnel arrived to find Obette Reyes Lacap, a 33 year old resident of Beaumont, unresponsive. He was pronounced dead at the scene at 11:55 AM. The incident is under investigation by the San Bernardino County Sheriff Homicide Detail.[05102013 0734 SY]

  16. Coroner case #701303552--On 5/9/13 at 2:42 AM, Corrina Marie Wilson, a 22 year old resident of Ontario, was traveling westbound Interstate 10 west at Riverside Avenue in Rialto in a 2007 red Nissan Altima. For an unknown reason the vehicle left the roadway and overturned. Wilson was pronounced dead on scene. The California Highway Patrol is investigating this incident.[05092013 1208 SY]

  17. Coroner case #701303549--On May 8, 2013 at approximately 9:48 P.M. San Bernardino Police officers responded to the 2100 block of Cajon Blvd. Officers found Billy Ray Duncan Jr, a 33 year old resident of Apple Valley, down with upper body trauma. Duncan was prounced dead at the scene by paramedics. The San Bernardino Police Department Homicide Detail is investigating this incident. [05092013 1145 SY]

  18. Coroner case #701303529: On 05/07/2013 at 2:27 PM, a 911 call was received reporting shots fired at an apartment complex located in the 1500 block of Lilyhill Drive in Needles. Deputies from the Colorado River Station responded and located Robert Bennett, a 72 year old resident of Needles, near the door of his apartment with upper body trauma. He was pronounced dead at the scene. The San Bernardino County Sheriff's Department, Homicide Division is investigating the incident. [051213 1050 TC]

  19. Coroner Case #701303508 - On 05/07/13 at 7:44 AM, 911 calls were received by various law enforcement agencies regarding a head-on collision that had occurred between an empty Colton Unified School District bus and a silver, GMC, pickup. Officers and deputies from various agencies responded to the location discovering that Melissa Ann Smith, a 34 year-old resident of Colton, was traveling southbound on Mt. Vernon Ave. north of Grand Terrace Rd. in Grand Terrace. The school bus was traveling northbound. For unknown reasons, Smith's vehicle veered in front of the school bus and the vehicles collided. Paramedics responded to the scene from the Colton Fire Department. At 7:54 AM, Smith was pronounced dead at the scene. Deputies from the San Bernardino County Sheriff Department Major Accident Investigation Team are conducting the investigation into the collision.[05072013 1435 SY]

  20. Coroner Case # 701303477---- On 05/05/2013, at 11:54 AM, San Bernardino County Sheriff’s Department deputies responded to a 911 call for a train versus a pedestrian in the area southwest of Swarthout Canyon Road and Southern Pacific in the Cajon Pass north of Devore. Deputies arrived on scene and found Ricardo Rosales, a 33 year-old male resident of Fontana, dead of his injuries. An autopsy will be conducted to determine the cause of death. The San Bernardino County Sheriff’s Department and Union Pacific Railroad Police are investigating the incident. (05062013 CN 1853)

  21. Coroner Case # 701303444---- On 05/03/2013, at 9:09 PM, California Highway Patrol officers responded to 911 calls for a auto versus a pedestrian in the area of eastbound Interstate 10 and Cedar Ave, in Fontana. Officers arrived on scene and found Michael Hampton, a 50 year old male resident of Montclair, dead of his injuries. An autopsy will be conducted to determine the cause of death. The California Highway Patrol is investigating the incident. (05052013 CN 1904)

  22. Coroner case #701303433: On Friday, May 3, 2013, at about 9:20 AM, the Chino Police Department received 911 calls reporting a man down to the front of a residence on the 12000 block of Santa Ana Ave. in Chino. When officers arrived at the residence, they found Robert Alejandro Hernandez, a 23 year-old male resident of Chino, down with upper body trauma. He was transported to the Kaiser Ontario Medical Center for treatment. He succumbed to his injuries at 11:47 AM in the hospital. The Chino Police Department Homicide Unit is handling the investigation. An autopsy will be conducted next week to determine the exact cause of death. [05042013 0923 GB]

  23. Coroner case # 701303435: On Friday, May 3, 2013, James Oliver Bolla, a 27 year old resident of Yorba Linda was involved in a motor vehicle collision on the eastbound Interstate 10 Freeway in Pomona. At about 12:06 PM Bolla lost control of the 2004 Toyota Tacoma pickup truck he was driving, struck a big-rig truck, then rolled off the roadway at the White Ave. off ramp. He was flown by Los Angeles County Fire to Arrowhead Regional Medical Center, where he was pronounced dead in the emergency room from his injuries at 2:29 PM. The collision is under investigation by the California Highway Patrol. [05042013 0923 GB]

  24. Coroner Case # 701303444---- On 05/03/2013, at 9:09 PM, California Highway Patrol officers responded to 911 calls for a auto versus a pedestrian in the area of eastbound Interstate -10 and Cedar Ave, in Fontana. Officers arrived on scene and found Michael Hampton, a 50 year old male resident of Montclair, dead of his injuries. An autopsy will be conducted to determine the cause of death.   25. Coroner Case # 701303447--On 05/03/2013, at 10:42 PM, Gloria Diaz, age: 66 from Long Beach was a passenger in a 2006 Ford Escort that was eastbound in the number none lane of Interstate 10 east of Cherry Avenue in Fontana. Traffic suddenly came to a stop due to a traffic collision several miles ahead when the Ford escort was struck behind by a 1995 Chevy Silverado. After impact the driver and passenger in the Silverado ran away on foot. Paramedics responded and transported Gloria to Kaiser Hospital in Fontana where she was pronounced dead at 11:27 PM. An autopsy will be conducted to determine the cause of death. The California Highway Patrol is investigating the incident. [05042013 0625 EM]

  26. Coroner case 701303414: On Thursday, May 2, 2013, Anthony Ramirez, a 23 year old resident of Upland, was driving his motorcycle eastbound on State Route 60 when a white van was observed to cut him off, possibly striking his motorcycle. He lost control of the bike, which then collided with the back of a pick-up truck at 4:40 PM in the HOV (high occupancy vehicle) lane, sustaining multiple injuries. Ramirez was transported by ambulance to Arrowhead Regional Medical Center, where he was pronounced dead in the emergency room at 5:41 PM. The accident is under investigation by the California Highway Patrol (CHP). [05/02/13 2300 VN]

  27. Coroner Case 06-9509DC--The San Bernardino County Sheriff's Department-Coroner Division is asking for public assistance. Back on 11/30/2006 Thomas Leroy Barton age 65, died of natural causes at San Antonio Community Hospital. He did not list anyone as his next of kin. He had also used the name of Roy Barton Thomas. He is believed to have been a transient in the Ontario area since 1998. Anyone with information as to his family is urged to contact the Coroner’s Division 909-387-2978. [120906 2025 RK]

  28. Coroner Case #06-9277CW—The Coroner’s Division of the San Bernardino County Sheriff’s Department is seeking the assistance of the media and public in locating the next of kin for Gilbert Manuel Guzman, age: 53, (DOB:09/01/1953). Guzman died of apparent natural causes, on December 1, 2006, at a friend’s home in the 4000 block of Linson Street, in the city of Adelanto. Guzman is believed to have lived previously in the Los Angeles area. Anyone with information is asked to call The Coroner’s Division at (909) 387-2978. [120206 2255 EM]

  29. Coroner Case #06-8824WB—The Coroner’s Division of the San Bernardino County Sheriff’s Department is seeking the assistance of the media and public in locating the next of kin for Enrique Valenzuela, age: 53, (DOB: 4/21/1953). Back on 11-14-06, he was found dead of apparent natural causes at his home in the 2100 block of W. Ogden Street, San Bernardino. Valenzuela is believed to have a daughter, Xochitl Valenzuela, living in Compton, California. Anyone with information is asked to call The Coroner’s Division at (909) 387-2978. [112906 2227 EM]

  30. Coroner case 06-8806 ML: Back on Tuesday, November 14, 2006, at 1:12 PM, a gardener clearing brush behind a business in the 1300 block of E. D Street saw a body near the back fence of the property and contacted the Ontario Police Department. Officers responded to the scene and confirmed the presence of a dead male adult with upper body trauma. The Ontario Police Department Homicide Detail is investigating the circumstances of the death. UPDATE (11/22/06): All attempts to identify the decedent have been unsuccessful and he has been designated John Doe #36-06. The body is described as being that of a Hispanic male, appearing to be in his late teens to twenties, 5’4”, 113 lbs, with short black hair. He had multiple tattoos, including the following: A sunburst design with a heart in the middle to the upper right chest, a mask (clown?) to the upper left arm, "Mikdalia" to the left forearm, and a skull and crossed bones to the upper right arm. One finger on his left hand had been amputated at the knuckle. Some people in the area knew him as “Marco Antonio Aguello Perez,” possibly from Chiapas.
