  1. Coroner Case # 701203735- On 05/24/2012 at approximately 7:00 AM, the Fontana Police Department responded to a shots fired call in the area of Frankfort Street and Dorsey Way in Fontana. When Officers arrived they found a 26 year-old male with upper body trauma. The male was transported to Kaiser Hospital where he was pronounced dead at 7:50 AM due to his injuries. The Fontana Police Department is investigating. The name of the decedent is being withheld pending positive identification and notification of next-of-kin. [05242012 1511 SY]

  2. Coroner case #701203710 – On 5/23/2012 at approximately 6:46 AM Stephanie M. Perez, a 32 year old female resident of Hesperia, was the driver and sole occupant of a 2005 Chevrolet Tahoe traveling northbound on Mariposa Road in Hesperia. At Mesa Street Perez drifted off the roadway, overcorrected and the vehicle rolled over off the roadway. Perez, who was not seatbelted, was ejected from the vehicle. She was pronounced dead at the scene. Hesperia Police is investigating this incident. [05242012 0707 SY]

  3. Coroner Case #701203708 - On 05/23/2012, at 1:07 A.M., Edward Garcia Urena, a 40 year old resident of Highland, was found down in a vacant lot, in the 900 block of E. San Jacinto St. in San Bernardino. Urena had sustained trauma to the upper torso and was pronounced dead at the location. The San Bernardino Police Department Homicide Detail is investigating.[05232012 1501 SY]

  4. Coroner Case #701203705 -- On 5/22/2012 at 10:32 PM, a 22 year old female resident of Redlands, was driving her 1997 Honda Civic northbound on Bryant St. at Topaz St., when the Honda drifted into the southbound lane for an unknown reason. A southbound 2004 Toyota Avalon was unable to avoid a collision and struck the Honda head-on. The female was extricated and transported to Loma Linda Univeristy Medical Center where she was pronounced dead at 12:25 AM on 5/23/12. When her next of kin has been notified, the name will be released to the public. The San Bernardino Sheriff Major Accident Investigation Team is investigating the collision.   5. Coroner Case number 701203691-- On 05/22/2012, at around 0900 hours, the body of a male adult was found in the backyard of a home in the 200 block of E. 11th Street, in the San Bernardino. When the identity is confirmed and the next of kin is notified, the name will be released to the public. The San Bernardino Police Department is investigating this death. (05232012 CN 0113)

  6. Coroner case #701203682 – On 5/22/2012 at 5:15 AM Mariano Orendain, a 50 year old resident of Kingman, AZ was the seat-belted passenger of a 2004 Dodge 1500 pick-up truck traveling westbound on 1-40 in Ludlow. At 1.5 miles west of Crucero Road the driver of the Dodge pick-up lost control of the vehicle for an unknown reason and it overturned across both westbound lanes and off the right (south) shoulder. The driver was transported by medical helicopter for treatment. Mr. Orendain was pronounced dead at the scene of his injuries. The Barstow California Highway Patrol office is investigating the incident. [05222012 1130 SY]

  7. Coroner case #701203644--On May 20, 2012, at 3:03 PM, Emergency Medical Personnel were called to Silverwood Lake for the report of a drowning male. Upon arrival they found Resendo Garcia, a 35 year old resident of Bell, had been pulled from the lake by on-lookers. Garcia was transported to Loma Linda University Medical Center by helicopter. Garcia was declared dead on May 21, 2012, at 0052 hours due to injuries. The San Bernardino County Sheriff Coroner will be conducting an autopsy to determine cause of death. (05212012 CN 0432)

  8. Coroner case 701203623: On Saturday night, May 19, 2012, at 9:39 PM, a single-vehicle, rollover traffic accident was reported to the San Bernardino County Sheriff Department. Deputies and San Bernardino County Fire personnel arrived at the scene, located at Koala Rd., just north of Auburn, in Adelanto. A green 1998 Ford Expedition had been traveling northbound along Koala at a high rate of speed had rolled into the desert, ejecting the adult male driver and two children, ages 7 and 2. Paramedics transported the two children to hospitals in the San Bernardino Valley. The driver, Armando Martinez, a 25 year old resident of Adelanto, was pronounced dead at the scene.   9. Coroner case #701203619--On May 19, 2012, at 7:35 PM, officers from the California Highway Patrol were sent to the area of Glen Helen Road, west of Cajon Boulevard, for a single vehicle motorcycle accident. When officers arrived they found the vehicle had lost control and struck a cement K-rail on Glen Helen Road, near the Burlington Northern Railway tracks. Tawny Mizer, a 29 year old resident of Upland, was ejected suffering head trauma. Mizer was taken by ambulance to the Saint Bernardine Medical Center emergency room where she was declared dead at 8:40 PM. The California Highway Patrol is investigating the collision.   10. Coroner case # 701203570--On 5/18/2012, at 7:33 a.m., The Ontario Police Department and Ontario Fire Department were dispatched to the BMW Training Center in the 1100 block of S. Dupont Ave. for an industrial accident. Phillip Weeks, a 48 year old resident of Rancho Cucamonga,had been changing the fluorescent emergency ballast system in the attic of the lobby at the business when he was electrocuted. Weeks was pronounced dead at the scene. The Ontario Police Department, the San Bernardino District Attorney's Office and Cal OSHA are investigating this accident.[05182012 1514 SY]

  11. Coroner case #701203569 – On 05/18/2012 at approximately 7:25 AM, the Chino Police and Fire Departments responded to a single vehicle traffic collision in the 3800 block of Philadelphia St., in the city of Chino. When officers and paramedics arrived they found a 64 year-old male unresponsive in the vehicle. The initial investigation revealed that the male was traveling eastbound on Philadelphia St., when for unknown reasons he lost control of his vehicle, struck the right shoulder curb and a street sign. The male was transported to Chino Valley Medical Center where he was pronounced dead at 8:03 AM. The name of the decedent is being withheld pending notification of next-of-kin. The Chino Police Department is investigating and an autopsy will be performed to determine the exact cause of death.   12. Coroner case 701203554-- Back on Wednesday, May 16, 2012, just after midnight, Jonathan E. Maxwell, a 30 year old resident of Riverside, was a passenger in a vehicle traveling along Iowa Ave. just south of Main St. in the city of Riverside, when another vehicle collided head on. Maxwell was transported by ambulance to Loma Linda University Medical Center, where he was pronounced dead late Thursday evening. The accident is under investigation by the California Highway Patrol.[05182012 1015 SY]

  13. Coroner Case #701203528 - On 05/17/2012 at about 7:34 AM, Joaquin De La Camara, a 54 year old resident of Riverside, was riding his Yamaha motorcycle northbound I-15 and was transitioning to the westbound I-10 in Ontario. De La Camara lost control of the motorcycle and collided with another vehicle. After colliding with the vehicle, De La Camara continued to the right shoulder where he struck the guardrail ejecting him off of the motorcycle and to the ground approximately 58 feet below. De La Camara was pronounced dead at the scene by paramedics at 7:46 AM. The California Highway Patrol is investigating the collision. [05172012 1400 SY]

  14. Coroner Case #701203521 - On 05/17/12 at 12:31 AM, Deputies from the Victor Valley Station were sent to the 14800 Blk of La Brisa Rd. in Victorville for a dispute between parties inside of a residence. Upon arrival a 60 year-old female resident of Victorville was found with upper body trauma. She was transported by ambulance to Desert Valley Hospital where she was later pronounced dead at 1:47 AM. The decedent's name will be released when the next-of-kin have been notified. An autopsy will be conducted next week to determine the exact cause of death.   15. Coroner case # 701203445--On 5/13/2012, at 9:44 a.m., San Bernardino Police Department and paramedics were dispatched to the 200 block of South Lenore Ave. in the city of San Bernardino reference a male unresponsive in a vehicle. Michael Espino, a 21 year old resident of San Bernardino, had upper body trauma and was pronounced deceased at the scene. San Bernardino Police Homicide detectives are investigating this incident.   16. Coroner case # 701203444 -- On 05/13/12, at 10:25 am, Harout N. Chouljian, a 59 year old male resident of Glendale, died on the west portion of the El Mirage dry lake bed in El Mirage. Chouljian was the sole occupant piloting his ultra light aircraft when, for an unknown reason, lost control of the aircraft. An autopsy was ordered to establish the cause of death and any other contributing factors. San Bernardino Sheriff Department (SBSD) – Aviation Division is investigating the accident.   17. Coroner case 701203412: On Friday, May 11, 2012, at approximately 6:25 PM, David Preston, a 27 year old resident of Yucaipa, was shot in the 22000 block of Deberry Street in Grand Terrace. He was transported by ambulance to Loma Linda University Medical Center emergency room, where he died of his injuries at 7:30 PM.   18. Coroner case # 701203398--On Thursday, 5/10/2012 at approximately 4:00 PM, a hiker flagged down the San Bernardino County Sheriff's Department Resident Deputy Sheriff out of Baker in the area of Nipton Road and Interstate 15. The hiker directed the deputy to where a human skull was found in the Mojave National Preserve. On Friday, 5/11/2012, the Sheriff's Homicide Detail and Coroner Division responded to the scene to conduct the investigation. The remains do not appear to be historical. An examination will be conducted at a later date for possible cause of death and identification.   19. Coroner case #701203389 – On 05/10/2012 at 3:00 pm the San Bernardino County Sheriff’s department Communications Division received a 911 call of a traffic collision on Avenue E just east of the Peach Ave intersection in Hesperia. Deputies arrived on scene to find a white 1999 Chevy Camaro and a gold 1995 Cadillac had collided. The driver of the Camaro was identified as Jonathan Plavney, age 19 of Hesperia. Plavney was pronounced dead on scene at 3:18 pm. The MAIT Division of the San Bernardino County Sheriff’s department is investigating the incident. [05112012 0755 SY]

LATE ENTRY--Coroner case # 701203318--On 5/7/2012 at 8:27 p.m. San Bernardino Police Department and paramedics were called to a residence in the 700 block of North Pershing Avenue in San Bernardino for a male stabbing victim. David Jamieson, a 51 year old resident of the home was transported to Loma Linda University Medical Center where he succumbed to his injury at 10:00 p.m. San Bernardino Police Homicide Detectives are investigating this incident.   21. Coroner Case #701203330-On Tuesday, May 8, 2012 at approximately 8:26 a.m., deputies from Victorville Station responded to a suspicious circumstances call in the area of Hesperia Rd. near Sunset Ave. The caller reported dead body was found at that location. Deputies arrived on scene and confirmed there was a deceased adult male, white or Hispanic, approximately 30 to 50 years of age. The deceased was dressed in female clothing, with a tattoo of a diamond above each eyebrow and a tattoo on his right cheek that appears to be a number. Due to the location and condition of the body, Sheriff's Homicide Investigators were called and are conducting a death investigation at this time. An autopsy will be conducted at a later date to determine to cause of death.   22. Coroner case #701203310 – Juanita Mullin, a 59 year old female resident of Las Vegas, NV, was the driver and sole occupant of a 2012 Chevy Impala who was northbound on I-15, 4 miles north of Baker. A semi tractor-trailer rig rearended the Impala. Mullin died at scene on 05/07/12 at 10:46 am. California Highway Patrol - Barstow is investigating. (05072012 CN 2031)

  23. Coroner case #701203303 – Michael P. Johnson, a 47 year old male resident of Apple Valley, was the driver a 2005 Harley Davidson westbound on D Street at Second Street, Victorville, when a eastbound Ford mini-van made a left turn in front of Johnson. Johnson struck van and was ejected from the motorcycle. Johnson was transported to Victor Valley Community Hospital and was pronounced dead on 05/07/12, at 05:26 am. This incident is being investigated by Victorville Police Department. (05072012 CN 2028)

  24. Coroner case #701203298 / 701203299 -- On 05-06-12, at approximately 9:17 PM, Officers of the San Bernardino Police Department responded to the Dellman Heights Recreational Center, 2696 N. Flores Street, for the report of possible gunfire. The investigation revealed that two men had sustained upper body trauma and had been taken to a local hospital by private vehicle. Both men, later identified as Corey Broussard, a 31 year old resident of Victorville and Ramiro Aguilar, a 35 year old resident of San Bernardino, had succumbed to their injuries and were pronounced dead.   25. Coroner case 701203296 VN: Back on Saturday morning, May 5, 2012, at about 2:00 AM, a man received electrical burn injuries while attempting to remove copper wiring from an electrical relay station on Stoddard Ave., just south of Rialto Ave., in the city of San Bernardino. Michael Morga Garcia, a 26 year old resident of San Bernardino, was transported by ambulance to Arrowhead Regional Medical Center, where he was pronounced dead Sunday evening, May 6th, at 8:32 PM.   26. Coroner case #701203245: On Friday, May 4, 2012, at 3:03 PM, the California Highway Patrol (CHP) received an emergency call reporting a motorcycle accident at Kelbaker Road and Kelso Cima Road in Kelso. CHP officers arrived on scene and found that Dennis Duke, a 68 year old resident of Torrance, had lost control of his motorcycle and was ejected onto the roadway. Paramedics from the San Bernardino County Fire Department arrived on scene transported him to a nearby landing zone where Mercy Air Ambulance paramedics were waiting. However, the patient was pronounced dead at 3:00 PM, soon after arrival at the landing zone. [05/04/12 2300 VN]

  27. Coroner case # 701203239 -- On 05/04/2012, at approximately 05:47 AM, the San Bernardino Police Department responded to a "shots fired" call in the general area of the 2000 block of Western Ave. in San Bernardino. Officers were unable to locate any indication of a shooting. At 6:15 AM, officers were dispatched to the alley of the 2000 block of Western Ave. reference a person down. When officers arrived they found Djuna Benjamin, a 48 year old resident of Rialto, with upper body trauma. Benjamin was pronounced dead at the scene.   28. Coroner case # 701203213 -- On 05/02/12 at 10:51 pm the San Bernardino Police Department received a 9-1-1 call of a single vehicle accident. David Stoupakis, a 51 year old male resident of Devore, was the driver of a 1996 Cadillac which was traveling southbound on Cajon Blvd. at a high rate of speed. Stoupakis struck the trailer of a semi-truck. When paramedics arrived Stoupakis was pronounced dead at the scene. The San Bernardino Police Department is investigating this incident.   29. Coroner case # 701203196 -- On 05/02/2012, at approximately 6:00 AM, the San Bernardino Police Department responded to a "shots fired" call in the 700 Block of N. San Anselmo Ave., in San Bernardino. When officers arrived they found Askia Salahuddin, a 68 year old resident of Rialto, with upper body trauma. Salahuddin was pronounced dead at the scene at approximately 6:30 AM. The San Bernardino Police Department is investigating. [050212 1258 TC]

Coroner case #701203164 - On 04/30/2012 at aproximately 9:45 PM, a vehicle traveling northbound on Fort Irwin Rd, just south of Kolath Rd., lost control and crossed into oncoming traffic striking a Honda Civic traveling southbound on Fort Irwin Rd., in Fort Irwin. Jonell Davis, a 24 year old resident of Fort Irwin, was pronounced dead on scene at 10:06 PM. The California Highway Patrol is investigating the incident.   31. Coroner case 701203141 - The San Bernardino Coroner is seeking the public's assistance in locating family for Jerome Lee Davis, a 59 year old Black male resident of the San Bernardino / Rialto area. He died on April 29, 2012 at 10:06 PM at Loma Linda University Medical Center of natural causes. He has a possible sister-in-law named of Sherilee Peace and possibly a son named Noah Davis.   32. Coroner case # 7012030: On Friday night, at 9:09 PM on April 27, 2012, DeJuan De Mario Byrd, a 35 year old resident of Long Beach, was struck by a westbound MetroLink commuter train as it approached the MetroLink station in Rialto. The collision occurred at the North Sycamore Ave. crossing. The San Bernardino County Sheriff Department - Coroner Division is searching for family.   33. Coroner Case # 701203104/701203106---On 04/28/2012 at 11:00 AM, San Bernardino Police Department responded to a call for shots fired in the 1200 block of N. Mountain View Ave, in the city of San Bernardino. Officers found Daniel A. Aguilar, a 39 year old resident of Fontana and Rodolfo Salazar Trujillo, a 29 year old resident of Rialto with trauma to the upper body. Both men were pronounced dead on scene by paramedics.   34. Coroner case #s 701203095 and 701203096-- On 04/28/2012 at 10:27 AM, a 911 call was received by the California Highway Patrol reporting a single vehicle rollover collision 1/4 miles south of mile marker 168 on the southbound lanes of Interstate 15 near Mountain Pass. Sonya De Anda Martinez (age 15) and Jesus De Anda Martinez (age 6) of Las Vegas Nevada were both pronounced dead at the scene. Their parents were taken to University Medical Center in Las Vegas for treatment of their injuries. Witnesses stated they saw the vehicle, a 1998 Ford Explorer traveling southbound on I-15 at approximately 70 miles an hour. The vehicle began to swerve and left the roadway to the west rolling multiple times coming to rest on its wheels approximately 200' from the roadway.   35. Coroner case #701203084--On 04/27/2012, at approximately 10:04 PM, officers from the California Highway Patrol observed a vehicle parked east of the I-15 freeway near mile marker 147.11, approximately seven miles north of Baker. Upon investigation a Black male adult was found with apparent upper body trauma. The male was declared dead on scene at 10:12 PM, by assisting San Bernardino County Sheriff Department personnel. The name of the individual is being withheld pending notification of next of kin. An autopsy will be performed by the San Bernardino County Sheriff Coroner to determine cause of death.   36. Coroner case 7012030 VN: On Friday night, April 27, 2012, at 9:09 PM, DeJuan De Mario Byrd, a 35 year old resident of Long Beach, was struck and killed by a westbound MetroLink commuter train as it approached the MetroLink station in Rialto. The collision occurred at the north Sycamore Ave. crossing. The incident is under investigation by the Rialto Police Department. [04/28/12 0800 VN]

  37. Coroner case 701203053 VN: Back on Monday, April 16, 2012, at about 7:45 PM, Zenaida Alba Hernandez, a 53 year old resident of Hesperia, was crossing Bear Valley Rd. on foot at the 5th Ave. intersection, when she was struck by a westbound motorcycle that had lost control and slid into the intersection. Hernandez was flown from the scene by Mercy Air Ambulance to Loma Linda University Medical Center emergency room where she was treated for head injuries. Surgeries were performed, but the patient never regained consciousness. On Thursday, April 27, 2012, Hernandez was pronounced dead at 10:34 PM.   38. Coroner cases # 701203007 & 701203010--On 04/25/2012 at approximately 01:58 AM, Anthony Allen (701203007), a 20 year-old resident of Corona was driving a 2005 Honda Accord westbound Foothill Blvd. west of Almeria Ave. in the city of Fontana. Allen crossed over into the eastbound lanes of Foothill Blvd. and struck a 1989 Toyota Corolla head on that was traveling eastbound. The driver of the Toyota, Rhonda Martinson (701203010), a 42 year-old resident of Fontana was pronounced dead at the scene at 02:06 AM. Allen was transported to Loma Linda University Medical Center where he was pronounced dead at 05:23 AM. The Fontana Police Department is investigating the incident. [04262012 0707 GB]

  39. Coroner case # 701203003 - At 9:41 PM the California Highway Patrol responded to a 911 call involving a single vehicle accident on the northbound 215 freeway north of Center St., in Riverside. Jerry Garcia, an 18 year old male resident of Moreno Valley, was the passenger of a 1995 Pontiac Grand AM. For an unknown reason the vehicle traveled off the freeway, onto the shoulder, struck a tree and rolled over, coming to rest on the roof. Upon paramedics arrival, Garcia was transported to a local hospital where he was pronounced dead of his injuries at 10:24 PM. The California Highway Patrol is investigating the incident.   40. Coroner case 701203004..On 4/24/2012, at approximately 10:19 PM, a 2002 Honda Accord traveling southbound on Escondido Ave. struck a bicyclist, who was crossing Escondido Ave. from east to west, just north of Hollister St.in Hesperia. Ryan D. Foster, a 34 year old resident of Hesperia, died at the scene from injuries sustained in the collision. The San Bernardino Sheriff Dept. Hesperia station is investigating the collision.   41. Coroner case #701202998 – Henry Penny, a 61 year-old resident of Arizona, died on 04/24/12, at 6:47 PM, on Interstate 40, east of the Hector Road off ramp, in Newberry Springs. Penny was the driver of a 1999 Dodge truck traveling eastbound on I-40 when he left the roadway for unknown reasons and the vehicle overturned. This incident is being investigated by the California Highway Patrol (CHP). (04292012 CN 2255)

  42. Coroner case #701202939 – Jose L. Contreas Jr., 20 year-old male of Victorville, died on 04/22/12, at 6;43 AM, on Interstate 15 at the Oak Hills Road off ramp in Hesperia. Contreas was the driver and sole occupant of a 2001 Chevrolet Impala northbound on I-15 when he lost control in the center divider, crossed all N/B lanes and struck a parked fire truck that had responded to another traffic accident in the same location moments prior. This incident is being investigated by California Highway Patrol (CHP) – Major Accident Investigation Team (MAIT) Riverside Division. [042312 0800 TC]

  43. Coroner cases 701202922 and 701202924: On Saturday, April 21, 2012 at 2:03 PM, a 911 call was received by the California Highway Patrol, reporting a traffic collision on Cajon Blvd. north of Ogden Street in the area of San Bernardino known as Muscoy. Paramedics from the San Bernardino County Fire Department arrived and found two Hispanic males, both to be estimated to be between 25 and 35 years of age, dead inside a 2004 Toyota Camry that was traveling southbound when it lost control for an unknown reason and was struck broadside by a northbound 2006 Honda Pilot. The names of the decedents will be released once identity has been established and next of kin have been notified. When deputies and paramedics arrived, they found that Gallegos was suffering from upper body trauma. Gallegos was transported to Desert Valley Hospital where he was pronounced dead at 2:49 PM. Coroner case 701202895--On Friday, April 20, 2012, just before 2:00 PM, Roy Ezell Davis, a 24 year old resident of Compton, was one of several masked suspects who entered the Fontana Swap Meet, an indoor shopping mart located in the 9700 block of Sierra Ave., in the city of Fontana. Coroner case #701202864 – On 4/19/2012 Jose Reyes, a 32 year old resident of Santa Ana, was a laborer working on jobsite lining a waste water pond in the 14000 block of Cougar Road in Helendale. Coroner case # 701202877--On 04/19/2012 at 4:50 PM, a 911 call was received by the California Highway Patrol, reporting the collision of a motorcycle and an automobile at the intersection of Almond Ave. and Arrow Route in Fontana. The motorcyclist, Sergio Moral-Perez, a 23 year old resident of Fontana, had been travelling west bound on Arrow Route and collided with an automobile making a left turn from south bound Almond Ave. onto east bound Arrow Route. When officers arrived they found that a 2002 Ford Expedition had been traveling northbound Interstate 15 south of Halloran Summit Rd. when the driver lost control of the vehicle, left the roadway into the center median, and rolled over multiple times. Coroner case #701202646--On 4/10/2012 at 6:05 p.m. San Bernardino Police Officers were called to respond to a call of skeletal remains found the wash area north of Highland Avenue, west of Highway 330 on Fontage Road Rd in San Bernardino. Coroner case #701202639 and 701202640—On Tuesday, 04/10/12 at approximately 9:27 am Highland Police deputies responded to a suspicious circumstances call at a residence in the 27200 block of Clifton Avenue. A male and female identified as Jesus Figueroa, a 59 year old resident of Highland, and Elizabeth Figueroa, a 55 year old resident of the residence were both found deceased from gunshot wounds. It appears that Jesus Figueroa arrived at the home of his estranged wife Elizabeth and shot her before shooting himself.   58. Coroner case #701202499 – On 4/5/2012 at approximately 8:12 p.m. a 2002 Cessna 182 crashed in the open desert just west of the Mojave National Preserve (20 miles north of Ludlow). The plane was located at 11:30 p.m. by San Bernardino Sheriff's Aviation personnel. Due to the condition of the aircraft, darkness and terrain, the decision was made to return the following a.m. to recover the remains of the pilot. Coroner case # 701202458- On 04/04/2012 at approximately 3:00 PM, a California Highway Patrol Officer attempted to conduct an enforcement stop for reckless driving on Joel Benge, a 48 year-old resident of Apple Valley, who was the driver of a Nissan Versa in the area of the 14200 Block Amargosa Rd. in Victorville. Benge failed to yield and a pursuit ensued. The pursuit terminated in the area of El Evado Rd. and Hopland St. where Benge exited the Nissan and fled on foot into the desert. Coroner Case #701202397 - On 04/01/12 at 10:07 PM, Officers from the Redlands Police Department were disptched to reports of a "motorcyclist down" on Lugonia Avenue east of California Street in Redlands. When they arrived, they found Christopher Lee Stanart, a 28 year-old resident of Yucaipa, down on the roadway. His black 2011 special construction "chopper" style motorcycle was nearby. It appeared that Stanart was eastbound on Lugonia Avenue east of California Street when he lost control of the motorcycle and left the roadway. At some point, Stanart was ejected from the motorcycle and struck the roadway. Paramedics from the Redlands Fire Department responded to the location and pronounced him dead at the scene at 10:18 PM. A male was driving a 1998 Ford Explorer westbound and for an unknown reason traveled into the the eastbound lane, hitting the Nissan head-on. Coroner cases 701202159 and 701202160 – On March 22, 2012 at 3:34 am, the San Bernardino County Sheriff’s Department Communication Division received a 911 call reference shots fired in the 8300 block if Via Ladera in Rancho Cucamonga. Deputies arrived to find Phyllis Harrington, age 42 resident of Rancho Cucamonga, with multiple gunshot wounds. Walter Jenkins, age 50, was also found in the home with a gunshot wound. Coroner case #701201748—On 3/8/2012 at 7:05 pm, Da’ Jion Sha-King Blie, a 14 year old resident of Bloomington was skateboarding west bound on San Bernardino Ave. west of Palm Lane in Bloomington when he was struck by a 2002 Toyota 4 Runner. Sha-King Blie was not wearing a helmet. American Medical Response and the San Bernardino County Fire Department responded and transported Sha-King Blie to Loma Linda University Medical Center where he was pronounced dead from his injuries at 11:22 pm. Coroner case #701201656 -- On Monday, March 5, 2012, at approximately 12:56 am, deputies from Morongo Basin Station responded to a report of an assault with a weapon call for service, in the 65500 hundred block of Twenty Nine Palms Highway. Deputies arrived on scene and attempted to contact the occupants of the home and were met with a single shotgun blast through the front door. Deputies secured the residence and requested Sheriff’s Specialized Enforcement Team. While SED was responding, the Sergeant on scene was able to talk the suspect out of the residence, and he was taken into custody without further incident. As deputies secured the residence they located Jennifer Bablot Aguilar, a 54 year old Resident of Joshua Tree, deceased with an apparent gunshot wound. Coroner case #701201645 and 701201646—On 3-4-12 at 10:12 pm, Ariyana Daniel, age: 5 and her brother, Thurgood Marshall, age: 9, both from Las Vegas, were passengers in a 2012 Nissan Altima travelling northbound on Interstate 15 north of Baker and collided into the rear of a tractor-trailer. Coroner Case #701201630-- On 03/04/2012 at about 1:50 AM, the San Bernardino Police Department received a call regarding a disturbance in the parking lot at the Pool Club at 1946 South "E" Street. When police arrived, they found 26 year old Andy Nath of San Bernardino had been assaulted. Paramedics transported him to Loma Linda University Medical Center where he was pronounced dead at 2:58 AM. Oien was struck by a vehicle that had been traveling westbound Old Woman Springs Rd. Oien was pronounced dead at the scene by paramedics at 6:20 PM. Coroner case #701201608 – On 03/02/2012 at approximately 5:40 PM, deputies from the San Bernardino Sheriff's Department responded to a motorcycle collision at Midway Rd. and Whitmore Rd. in Twentynine Palms. When deputies arrived they found that two off road motorcycles had collided broadside in the intersection. Paramedics arrived on scene and transported Tinoifili Maina, a 26 year old resident of Twentynine Palms to Hi-Desert Medical Center where after arrival he was pronounced dead from his injuries at 6:41 PM. Coroner case #701201582-–On 03/02/2012 at 12:49 AM, the California Highway Patrol received a 911 call reference a pedestrian struck by a car on eastbound I-10 at the Pepper Ave overpass in Colton. Police and fire fighters arrived to find a 21 year old transient from South Carolina dead on scene. The California Highway Patrol is investigating the incident.   97. Coroner case #701201544 – On 02/29/2012 at 7:46 pm, officers from the San Bernardino Police Department initiated a traffic stop at W. 16th Street just east of N. Arrowhead Ave., in the city of San Bernardino. Contact was made with the driver. The passenger, identified as Anthony Gilmore, age 23 of Rialto, fled from the vehicle on foot. A foot pursuit ensued and officers were able to catch up with Gilmore. A physical altercation ensued escalating into an officer involved shooting. Gilmore was transported to Loma Linda University Medical Center where he was pronounced dead at 8:52 pm. The San Bernardino Police Department is investigating the incident. [03012012 0049 GB]

  98. Coroner case #701201531-–On 02/29/2012 at 11:19 AM, the Barstow Police Department received a 911 call reference a person jumping off the Barstow Road overpass on to southbound I-15. Police and fire fighters arrived to find Derek Revette, age: 32, from Boyes Hot Spring, unresponsive and CPR was initiated. Paramedics transported him to Barstow Community Hospital where he was later pronounced dead at 12:22 PM. The Barstow Police Department is investigating the incident. [02292012 GB]

  99. Coroner case #701201508-–On 02/28/2012 at 2:16 PM, the San Bernardino Fire Department received a 911 call reference a fire at 1157 Tia Juana Avenue in San Bernardino. When fire personnel arrived, the residence was on fire and it was reported a man was still inside the home. Multiple firefighters entered the residence and extricated Steve Duncan, age: 55, from Rialto, who was unresponsive at which time CPR was initiated. Paramedics transported him to Arrowhead Regional Medical Center where he was later pronounced dead at 2:56 PM. An autopsy will be performd to determine the cause of death.   101. Coroner Case #701201479--On 02/27/2012 at 2:52 PM, the San Bernardino Police Department received a 911 call reference shots heard in the 1100 block of N. Wilson Street in San Bernardino. When officers arrived, they found Anthony Dewayne Mitchell, age 15, from San Bernardino had been shot. Paramedics transported him to Arrowhead Regional Medical Center where he was pronounced dead at 3:28 PM. An autopsy will be performed to determine the cause of death. Coroner case #701201451-- On Sunday, 02-26-12, at 2:38 AM, the San Bernardino County Sheriff’s Department received a 911 call by a passerby who advised that there had been a two vehicle collision at the intersection of Palmdale Road and Amethyst Road in the city of Victorville. When Deputies arrived, they found a 33 year-old White male resident of Victorville/Hesperia, that had been the passenger in a two door Honda that had been ejected from the vehicle. The drivers of both vehicles were flown to area hospitals. Paramedics responded to the location and confirmed the male dead at the scene at 2:48 AM. Coroner case #701201445--On Saturday, February 25, 2012 at 11:50 PM, the California Highway Patrol received a 911 call by a passerby who advised that a vehicle had struck a large parked Caterpillar loader on the westbound shoulder area of Interstate 10 west of Riverside Ave. in Rialto. When officers arrived, they found Michael Joseph Franco, a 38 year-old resident of Fontana, inside of a gray 2001 Chevrolet sedan. There was construction to build a cement retaining wall where the piece of heavy equipment was parked along the shoulder area of the freeway. Paramedics from the San Bernardino County Fire Department responded to the location and pronounced Franco dead at the scene at 12:06 AM on 02/26/12.   104. Coroner case # 701201395-- On 2/24/2012 at 6:56 A.M., Timothy Robert Slee, a 65 year old resident of Chino Hills, was the driver and solo occupant of his 2005 Honda Accord. Slee was traveling westbound on Fairfield Ranch Road near Red Barn Court in Chino Hills. For an unknown reason he began drifting over into the eastbound lanes. Slee's vehicle went up on a curb, sideswiped a tree and came to rest in some bushes. The vehicle's airbags deployed. Slee was was pronounced dead at the scene by the Chino Fire Department.   105. Coroner case #701201362--- On February 22nd, 2012 at 7:16 PM San Bernardino County Sheriff Department deputies responded to the 11500 block of Stoneridge Drive, in Rancho Cucamonga. Deputies found Hannah Kang, a 60 year-old resident of Rancho Cucamonga with upper body trauma. Kang was transported by air ambulance to Loma Linda University Medical Center. Despite medical interventions, Kang was pronounced dead at 7:10 AM on 02/23/2012. An autopsy will be conducted to determine the cause of death. Coroner case # 701201351 -- On 02-22-2012 at 5:40 PM, a 65 year old male resident of Phelan, was riding a Harley Davidson motorcycle westbound on Bear Valley Road, approaching Mesa View Road in Victorville. A blue Ford F150 pick up truck turned left in front of the motorcyclist and the two collided head on in the intersection. Paramedics responded to the 911 call and the motorcyclist was pronounced dead at 5:50 PM. The decedent's name will be released once next of kin is notified of the death.   107. Coroner case #701201229--- On 02/18/2012 at 2:06 am, California Highway Patrol officers responded to a 911 call reporting a traffic collision on westbound I-10, west of S. Riverside Ave, in the city of Rialto. A black 2007 Honda Civic was struck from behind by a 2002, silver Nissan Frontier pickup truck. Following the collision, the 22 year old male driver, a resident of Rialto got out of his Nissan and was running south bound across I-10 when he was struck by a tractor trailer traveling eastbound on I-10. San Bernardino County Fire Department paramedics confirmed the man dead from his injuries at 2:19 am. The name of the decedent will be released upon notification of the next of kin.   108. Coroner case #701201227 - On 02/17/2012 at 10:45 PM, in the Dumont Dunes off road vehicle area in rural Baker. Nicholas Sean Blanchette, age 24, a resident of Riverside was riding an ATV on the "Competition Hill" and struck a sand rail broadside at about 40 MPH. The ATV rider was ejected from his vehicle. Paramedics that were stationed in the area responded to the location and transported the driver to their first-aid station and then pronounced him dead at 11:25 PM.   109. Coroner case #701201209--- On Thursday, February 16th 2012 at 10:06 PM San Bernardino County Sheriff Department deputies responded to the 9000 block of Anza Trail, in Lucerne Valley. Deputies found Damian Negrete, a two year old resident of Lucerne Valley with trauma. Negrete was transported by ambulance to St Mary Medical Center, in Apple Valley, then by air ambulance to Loma Linda University Medical Center. Negrete was pronounced dead at 01:25 AM on 02/18/2012 at Loma Linda University Medical Center. An autopsy will be conducted to determine the cause of death.   112. Coroner case #701201063 -- On February 10, 2012 at 6:30 PM, Donna Joyce Clark, a 50 year old resident of Victorville, was walking northbound 6th St. at D St., in the city of Victorville. Clark was struck by a vehicle that was traveling eastbound on D St. as she tried to cross the street. Clark was transported to Victor Valley Community Hospital and was pronounced dead from her injuries at 7:39 PM. Coroner case #701201034 - On February 9, 2012 at approximately 8:16 PM, Danielle Gerhold, a 31 year old resident of Apple Valley, was driving a 2011 Nissan Versa eastbound on Red Feather Rd. in Apple Valley when she lost control of the vehicle causing it to overturn into the driveway of a residence in the 19100 block of Red Feather Rd. When deputies from the San Bernardino County Sheriff’s Department’s Apple Valley Station and paramedics arrived they found that Gerhold died from injuries suffered in the traffic collision at 8:28 PM.   114. Coroner case #701201017--On February 9th 2012 at 9:27 am, Donald Wilson, age 28, a resident of San Bernardino was found by California Highway Patrol officer’s supine on Cajon Boulevard at Mt. Vernon in San Bernardino. Wilson had apparently jumped off the West Highland Avenue overcrossing, after being involved in a traffic collision. He was transported by medics to the Community Hospital of San Bernardino where he was pronounced dead at 10:33 am. Coroner Case #701200999 - On 02/08/2012, at 8:46 PM., Dennis Kelly Timms, a 61 year old resident of Fontana, was walking in the westbound lane of the 13800 block of Arrow Route, in the city of Fontana. Timms was struck by a vehicle that fled the area in an unknown direction. Timms was pronounced dead at the scene. The California Highway Patrol is investigating this incident. [02092012 0701 SY]

Coroner Case #701200958--On 2-7-12 at 3:39 AM, the San Bernardino County Fire and Sheriff's Department responded to a fire located in the backyard of a residence located in the 9800 block of Carob Avenue in Fontana. When emergency personnel arrived, they found a parked motor home on fire. Once the fire was extinguished, the body of a male adult was found in the motor home. An autopsy will be performed to determine the cause of death. When identity has been determined and next of kin notified, his name will be released. Coroner case # 701200920 -- On 02/05/2012, at 8:48 PM, the San Bernardino County Sheriff Department responded to 911 call for an assault victim at a residence in the 15700 block of Sago Dell in the city of Hesperia. The victim, 18 year old Dennis Moore of Hesperia, was transported to a local hospital where he died from upper body trauma at 9:30 PM. The victim’s name will be released once next of kin has been notified. An autopsy will be conducted to determine the cause of death. Coroner case 701200919 -- On 02/05/2012, at 9:43 PM, 22 year-old Victorville resident, Devance Jerome Coleman, was walking northbound across State Highway 18, west of Bass Hill Road, in Apple Valley, when he was struck by several vehicles that were traveling westbound on State Highway 18. Coleman was pronounced dead at the scene, by paramedic personnel, from the San Bernardino County Fire Department.   119. Coroner case 701200915 VN: On Sunday, February 5, 2012, just after 3:00 PM, an accident was reported to the California Highway Patrol. A 1991 Toyota Camry traveling northbound along Central Ave., just north of Mission Blvd., in an unincorporated area of Montclair, left the roadway and collided with a tree. Paramedics responded to the scene and found the driver and sole occupant of the vehicle, Baltazar Prieto, a 58 year old resident of Upland, unresponsive. Prieto was flown by air ambulance to Loma Linda University Medical Center emergency room, where he died of his injuries at 4:36 PM. The accident is under investigation by California Highway Patrol.   120. Coroner Case #701200856 – at 8:54 am on February 3rd, 2012, Thomas Edgar Pippin, age 60 of Apple Valley, was found non-responsive in an alleyway in the 16000 block of Muni Road, Apple Valley. Paramedics were called and Pippin was pronounced deceased at scene. An autopsy was ordered to determine the cause of death and any contributing factors.   121. Coroner case #701200823 & 701200824 - On 2/1/12 at about 9:16 PM, the San Bernardino Police Department received a call of "shots fired" in the 1400 block of Sepulveda Ave., in San Bernardino. Officers arrived on scene to find two hispanic male adults with upper body trauma. Roberto Elizarraraz, a 19 year old resident of Highland, and Angel Perez, a 28 year old resident of Redlands, were pronounced dead on scene.   122. Coroner case #701200813 – On 02/01/2012 at 03:09 am, California Highway Patrol officers responded to a traffic collision on eastbound Highway 60, west of Central Ave, in Chino. Francisco Flores Jr, a 36 year old resident of Chino, was driving a 2006 Honda Civic when he collided with a parked semi truck and trailer. Coroner Case #06-9277CW—The Coroner’s Division of the San Bernardino County Sheriff’s Department is seeking the assistance of the media and public in locating the next of kin for Gilbert Manuel Guzman, age: 53, (DOB:09/01/1953). Guzman died of apparent natural causes, on December 1, 2006, at a friend’s home in the 4000 block of Linson Street, in the city of Adelanto.   126. Coroner case 06-8806 ML: Back on Tuesday, November 14, 2006, at 1:12 PM, a gardener clearing brush behind a business in the 1300 block of E. D Street saw a body near the back fence of the property and contacted the Ontario Police Department. Officers responded to the scene and confirmed the presence of a dead male adult with upper body trauma. The Ontario Police Department Homicide Detail is investigating the circumstances of the death. UPDATE (11/22/06): All attempts to identify the decedent have been unsuccessful and he has been designated John Doe #36-06. The body is described as being that of a Hispanic male, appearing to be in his late teens to twenties, 5’4”, 113 lbs, with short black hair. He had multiple tattoos, including the following: A sunburst design with a heart in the middle to the upper right chest, a mask (clown?) to the upper left arm, "Mikdalia" to the left forearm, and a skull and crossed bones to the upper right arm. One finger on his left hand had been amputated at the knuckle. Some people in the area knew him as “Marco Antonio Aguello Perez,” possibly from Chiapas.
