  1. Coroner Case #701200856 – at 8:54 am on February 3rd, 2012, Thomas Edgar Pippin, age 60 of Apple Valley, was found non-responsive in an alleyway in the 16000 block of Muni Road, Apple Valley. Paramedics were called and Pippin was pronounced deceased at scene. An autopsy was ordered to determine the cause of death and any contributing factors. The Coroner’s Division is seeking aid in locating the next of kin. Anyone with information is encouraged to call the San Bernardino Sheriff Department (SBSD) – Coroner Division (760) 243-8750.   2. Coroner case #701200823 & 701200824 - On 2/1/12 at about 9:16 PM, the San Bernardino Police Department received a call of "shots fired" in the 1400 block of Sepulveda Ave., in San Bernardino. Officers arrived on scene to find two hispanic male adults with upper body trauma. Roberto Elizarraraz, a 19 year old resident of Highland, and Angel Perez, a 28 year old resident of Redlands, were pronounced dead on scene. The San Bernardino Police Homicide unit are investigating the incident. (02022012 CN 0713)

  3. Coroner case #701200813 – On 02/01/2012 at 03:09 am, California Highway Patrol officers responded to a traffic collision on eastbound Highway 60, west of Central Ave, in Chino. Francisco Flores Jr, a 36 year old resident of Chino, was driving a 2006 Honda Civic when he collided with a parked semi truck and trailer. Flores was transported to Chino Valley Medical Center where he was pronounced dead at 03:38 pm. The California Highway Patrol is investigating the accident. (02012012 CN 0947)

  4. Coroner case #701200802 & 701200803 – On 01/31/2012 at 10:47am, California Highway Patrol officers responded to a traffic collision southbound I-15 between Palmdale Road and Bear Valley Road in Victorville. The investigation revealed that Marie Alessandro, an 86 year-old female resident of Victorville, was driving her 2007 Toyota Corolla northbound on the southbound side of I-15. She collided with a 1993 Cadillac Eldorado head-on. The Cadillac was driven by John Bushwalter, a 67 year-old male resident of Helendale. Bushwalter was pronounced dead on-scene at 11:48 am. Alessandro was transported to Arrowhead Regional Medical Center by helicopter where she was pronounced dead at 12:06 pm. The California Highway Patrol is investigating the accident. [013112 1745 TC]

  5. Coroner case #701200763 - Deputies from the Victorville Police Department were sent to the intersection of Bear Valley Dr. and Industrial Blvd. for a traffic collision on 01/30/12 at 1:00 AM. When they arrived at the location, they found that the driver of a 1993 Buick 4-dr had been traveling southbound on Industrial and was preparing to make a left turn onto eastbound Bear Valley. A second driver was westbound on Bear Valley approaching Industrial and apparently drove through a red light striking the Buick broadside. The driver of the Buick, 47 year-old Andrew Joseph Brown, resident of Apple Valley, was pronounced dead at 1:10 AM at the scene. The Victorville Police Department MAIT team is investigating the accident. [013112 1530 TC]

  6. Coroner case# 701200757-701200760 -- On 01-29-12, at 9:35 PM, A 1998 Chevy Suburban (with three brothers) was eastbound on Baseline Road and collided with a southbound 1998 Toyota 4-door sedan driven by a female adult at Cherry Avenue in Fontana. After the initial impact, the Suburban left the roadway and collided into a power pole on the southeast corner. Paramedics responded and pronounced two brothers (the driver and a rear passenger) in the Suburban dead at the scene. They have been identified as a 35 year old male from San Bernardino and a 39 year old male from Bloomington. The third occupant was transported to Kaiser Hospital and pronounced dead at short time later. He is a 37 year old male from Alta Loma. Noah Ferrer, age: 11 from Fontana, was a passenger in the Toyota. He was transported to Loma Linda University Medical Center where he died in surgery at 12:26 AM on 01-30-12. Autopsies will be performed to determine the cause of death. When next-of-kin has been notified and identities confirmed on the brothers, their names will be released. The Fontana Police Department is investigating the collision.   7. Coroner case 701200739 VN: On Friday, January 27, 2012, at about 7:15 PM, Steven Guzman, a 15 year old resident of San Bernardino, was walking in the 1500 block of north Bell Ave. (in San Bernardino), when he was fired upon from a passing automobile. Guzman was transported by ambulance to Loma Linda University Medical Center, where he died of his wounds on Saturday evening, January 28th.   8. Coroner Case #701200713 - On 01/27/2012 at approximately 5:47 PM, Holly Foor, a 42 year old resident of Crestline, was driving a 2005 Honda Civic northbound on State Route 18, between upper and lower Old Waterman Canyon Rd., at mile marker 11.9, in the city of San Bernardino. A driver of a 1997 Ford Explorer who was also driving southbound on State Route 18 crossed the double yellow line into the northbound lanes and struck the Honda head-on. Foor was transported by paramedics to St. Bernardine Medical Center and died from her injuries at 6:48 PM. The San Bernardino Police Department is investigating the collision. (01282012 CN 0030)

  9. Coroner case #701100662—On 1/26/2012 at 12:30 AM, Ronald Christopher Peters, age: 21, from San Bernardino, who was wanted for robbery, fled in a vehicle from law enforcement officers from the area of University Parkway and the 215 freeway in San Bernardino and was pursued to Highway 138 in Phelan. At about 1:13 AM, the pursuit ended and while being approached by law enforcement officers, he shot himself. Paramedics responded and pronounced him dead at the scene. An autopsy will be performed to determine the cause of death. The San Bernardino County Sheriff’s Department is investigating the incident. [01262012 1240 EM]

  10. Coroner case # 701200634 – On 01/25/2012, at 7:00 AM, 66 year-old Alfred Eclipse, Jr., of Norwalk, CA, was driving a UPS tractor trailer northbound Interstate 15, south of Oak Hills Road in Hesperia when he lost control due to another out-of-control vehicle which caused the tractor trailer to swerve off the roadway and rollover. Eclipse was pronounced dead at 7:20 AM by emergency personnel. The CHP is investigating the accident. [012512 1300 TC]

  11. Coroner case# 701200606 -- On 01/18/2012, at 6:48 PM, the San Bernardino Police Department received a 911 call of "shots fired" in the 2100 block of Chestnut Street. Officers responded to the Village Green Apartment Complex and found several males suffering from traumatic injuries. Twenty-seven year-old Gabriel Moraga of San Bernardino was taken to Arrowhead Medical Center where he later died of his injuries on 01/24/2012 at 7:58 AM. An autopsy will be performed to determine the cause of death. San Bernardino Police Department is investigating. [012512 1250 TC]

  12. Coroner case #7012000561--On January 22, 2012 at approximately 1:15pm , 45 year old Lance Black of Brea was found dead in the open desert of Johnson Valley about 150 feet off the motorcycle course where he had entered in an 80 mile race sanctioned by the Desert Motorcycle Club. Black was pronounced dead at the scene from his injuries. Black was an elite racer with many years experience. [01232012 2210 GB]

  13. Coroner case 701200569-- On Sunday, January 22, 2012, at 1112 PM, a 911 call was received by San Bernardino City Fire Dispatch reporting a residential structure fire in the 800 block of “G” St. San Bernardino police officers arrived at the scene first and found the building engulfed in flames. Officers attempted to gain access via the front door, a security door, which was locked. They opened the door, but were forced back by flames engulfing the interior of the apartment. San Bernardino Fire personnel arrived, extinguished the flames, and found a body within the apartment. The identity of the victim is withheld pending additional investigation. The incident is under investigation by San Bernardino Police Department and San Bernardino City Fire Department (SBFD). [01/23/12 0400 VN]

  14. Coroner case # 701200567- On Sunday, January 22, 2012 at 7:46 PM, Andy Boschma, a 48 year old resident of Ontario was driving a 2002 Chevy truck traveling eastbound on Merrill Ave in Ontario when he lost control of the vehicle and struck a tree. Boschma was transported to Arrowhead Regional Medical Center where he died from his injuries at 8:53 PM. The Ontario Police Department is investigating the collision. [01232012 0225 GB]

  15. Coroner case # 701200550 -- On 01/22/12, at 3:35 AM,the CHP received a 911 call reference a pedestrian that appeared to have been struck by a vehicle and was lying on the side of the roadway of San Bernardino Avenue east of Commerce Street in the city of Fontana. When emergency services arrived at 3:38 AM, a 44 year-old Huntington Beach resident was pronounced dead. The name of the decedent will be released to the public when the family has been notified of his demise. The CHP is investigating the hit and run accident.   16. 701200530 –On 1/20/2012 at 4:22 PM, Kevin E. Medina, a 25 year old Hesperia resident, was the driving a 2012 Ducati motorcycle southbound on Hesperia Road when an eastbound 1998 Toyota Camry entered Hesperia Road into the path of the motorcycle, resulting in a collision. Paramedics transported both drivers to area hospitals. Medina was pronounced dead on 01/20/2012 at 5:19 pm at Desert Valley Hospital in Victorville. The San Bernardino Sheriff Departmen (Hesperia Police) is investigating the incident. (01212012 CN 0043)

  17. Coroner Case #701200521 - On 01/20/2012 at approximately 11:00 AM, Leonor De Coronado (she also went by the name Lily Coronado), a 37 year old resident of Big Bear City, was driving a 2005 Kia Sorrento northbound on State Route 18, east of Paradise Way, in the unincorporated area of Big Bear City. Her 16 year-old daughter as a passenger. A Peterbilt dump truck pulling a trailer with a gravel box on it was traveling southbound. As the driver of the Peterbilt entered a sweeping left turn the trailer left the right shoulder striking the mountainside. The collision caused the gravel box to separate from the trailer’s frame and come to rest in the opposing lane. The trailer frame also continued into the opposing lane and struck the Kia. De Coronado was transported by paramedics to Bear Valley Community Hospital and died from her injuries at 12:12 PM. The 16 year-old daughter was airlifted to a local trauma center with moderate injuries. The California Highway Patrol is investigating the collision. (01212012 CN 0038)

  18. Coroner case #701200433 – On Monday January 17, 2012 at 12:13 pm, the California Highway Patrol was notified of a traffic collision on northbound Interstate 15 approximately a half mile north of the Duncan Canyon Road overpass in the city of Fontana. Officers arrived on scene and found that a 1995 Buick Park Avenue had struck a Department of Transportation Changeable Message Sign (CMS) pole. The preliminary investigation revealed The Buick was traveling northbound on Interstate 15. For an unknown reason the driver veered to the right and struck the CMS pole. The driver was transported to Arrowhead Regional Medical Center where he was pronounced dead at 1:08 pm. The driver was identified as Charles Gerald Jiles, age 58 of Rancho Cucamonga.   19. Coroner case # 701200392-- On Sunday January 15th 2012 at 5:09 pm Colton Police Department officers were dispatched to a disturbance call in the 1100 block of South Mohave Drive in Colton. An officer arrived on scene and encountered Hutalio Serrano, age 43, a resident of Colton who was not following the officer’s commands and actively resisted the officer. An electronic control device was deployed but was ineffective. It took three officers to successfully apply handcuffs. Serrano continued to resist after being handcuffed. Serrano was described as a large man weighing over 260 pounds who was exerting a great amount of energy while resisting the officers. While in custody Serrano suffered some type of medical emergency and paramedics responded to the scene for treatment. Serrano was subsequently transported to Loma Linda University Medical Center where he was pronounced dead at 6:23 pm. An autopsy will be performed later this week to determine the cause of death.   20. Coroner Case # 701200286--- On 01/11/2012, at 4:57 PM, San Bernardino County Sheriff Department responded to 911 call for a possible dead body in the area of 1st Avenue and Jasmine Street in the city of Victorville. Coy Glen Moses, a 50 year old transient, was found with upper body trauma. Paramedics confirmed Moses was deceased at 5:17PM. An autopsy will be conducted to determine the cause of death.   21. Coroner cases #701200158 and 701200159 – On 01/07/2012 at 3:25 AM, the California Highway Patrol received a 9-1-1 call reporting a traffic collision on Interstate 15 at Yermo Road in the city of Barstow. Officers arrived on scene to find that a 2004 Volvo XC90 had been traveling northbound on Interstate 15 and for an unknown reason, drifted off the paved surface and struck a support pillar at the Yermo Road overpass. Paramedics arrived and pronounced two passengers dead at the scene. The female passenger, 25 year old Erica Gale Niederhauser, and the male passenger 35 year old Paul Christian Despol, both of Glendale, CA were pronounced dead at the scene. The driver was transported to an area hospital for treatment. The California Highway Patrol is investigating the incident. [01102012 0924 SY]

  22. Coroner case #701200212-- On January 09, 2012 at approximately 3:16 AM, emergency personnel responded to a residential fire at 9151 Lemon Court, Fontana. When San Bernardino County Firefighters arrived they found the residence engulfed in flames. After the fire was extinguished, the body of Pablo Del Olmo, a 71 year old resident of Fontana, was found in the backyard of his residence with non fire related upper body trauma. The Hispanic male was declared dead on scene by San Bernardino County Fire personnel at 3:47 AM. The name of the victim is being withheld pending family notification. An autopsy will be performed to determine the cause of death. The Fontana Police Department Homicide Division, San Bernardino County Sheriff Department Arson Division and San Bernardino County Fire Department is investigating the incident. [01102012 0909 SY]

  23. Coroner case # 701200232--- On 01/09/2012 at 5:31 AM, Bernard F. Culverson, a 61 year old resident of San Bernardino was riding a bicycle in area of 3rd St and Waterman Ave in San Bernardino when he was stuck by a vehicle. Paramedics transported Culverson to Loma Linda University Medical Center where he died of his injuries on 01/10/2012 at 3:41 AM. The San Bernardino Police Department is investigating the collision.   24. Coroner case #701200187 -- On 01/08/2012, at 0310 hours, Joshua Daniel Henry a 28 year old resident of Fontana, was traveling westbound on Interstate 10 west at Cedar Avenue in the city of Fontana, in a gray 1997 Pontiac. For an unknown reason Henry's vehicle left the roadway and struck a tree. Henry was pronounced dead at the scene. The California Highway Patrol is investigating the accident. [010812 0730 TC]

  25. Coroner case 701200186 VN -- Early on the morning of Sunday, January 8, 2012, the body of a Black male adult was found lying in the roadway of Vineyard Ave., just south of E. 9th St. in Rancho Cucamonga. The man was confirmed to be dead at 0115 hours. It was apparent that he had been crossing the street (Vineyard) when he was struck by a southbound vehicle, which fled the scene (hit and run). The name of the victim is withheld pending identification and notification to the next-of-kin.   26. Coroner case # 7012000182 -- On 01/07/12, at about 8:30 pm, deputies from the Victor Valley Sheriff's Station received a call from 41 year-old Steve Emery from his residence in the 26300 block of Corona Road in Helendale. Emery said he may have shot one of five juveniles he believed were going to kill him. When deputies arrived at the residence they found that there were no juveniles and Emery had fired several rounds inside of his residence. Emery was taken into custody and a firearm was recovered. Shortly after being taken into custody, Emery became combative. Deputies restrained Emery and found he was having a medical problem. Emery was given medical aid by the deputies until paramedics arrived at the scene. Emery was transported to Victor Valley Hospital where he was pronounced dead. Investigators from the Sheriff's Department's Homicide Detail responded to the scene to conduct the death investigation. An autopsy will be conducted by the Riverside Coroner's Office to determine the cause of death. [010812 1720 TC]

  27. Coroner case# 701200172 -- On 01-07-12, at 2:33 pm, Francisco Huesca a 19 year old Milpitas, California resident was the non-seatbelted driver of a 1996 Toyota Corolla. Huesca was southbound on I-15, one mile north of Afton Road, in Newberry Springs when he lost control, crossed the open center divider, went airborne, and struck the side of a northbound big rig. Huesca's vehicle overturned, at which time he was ejected. The other three passengers had minor injuries and were transported to Barstow Community Hospital. This incident was also investigated by CHP. [010812 0730 TC]

  28. Coroner case # 701200162 -- On 01-07-12, San Bernardino Police had attempted to stop and speak with 32 year old San Bernardino resident Lorenzo Anthony Baca III, as he was walking on the 5th Street overpass in San Bernardino. Baca ran away, produced a handgun and began shooting at the officer who was chasing him. The officer returned fire and struck the male suspect. The male was transported to the Loma Linda University Medical Center Emergency Room where he was pronounced dead at 8:19 AM. [010812 0745 TC]

  29. Coroner case 701200126 VN: Just after midnight, Friday, January 6, 2012, a passerby driving along Fairway Dr., east of Sperry Dr., came upon a man lying in the roadway. Paramedics were called to the scene and the man was pronounced dead. The decedent was identified as Daniel Robert Wilkey, a 53 year old transient from Akron, Ohio. An examination will be conducted early next week to attempt to determine the cause of Wilkey’s death.   30. Coroner case #701200123 – On 01/05/2012 at 5:57 PM the California Highway Patrol received a 9-1-1 call of a a traffic collision on Highway 330 approximately 500 feet south of mile marker 37.75 (approximately 4 miles south of Live Oak Rd.) in the Running Springs area. Officers arrived on scene to find that a 2006 Pontiac Solstice was traveling northbound on Highway 330 when for an unknown reason veered into opposing lanes of traffic and was struck by a vehicle traveling southbound on Highway 330. The driver of the Solstice was identified as Donald Green, age 70 of Running Springs. Green was transported to LLUMC. He died on 01/06/2012 at 12:40 AM as a result of his injuries.   31. Coroner Case 701200085 - On 1/4/12 at 6:04 PM Oscar Dorado Padilla a 23 year old resident of Moreno Valley was struck and killed by a Metrolink train on S. Acacia Ave in Rialto. Padilla was pronounced dead on scene. Rialto Police Department and Metrolink officials are investigating the incident.[01052012 0835 SC]

  32. Coroner case #701200070-- On January 04, 2012 at 6:30 AM, the California Highway Patrol and San Bernardino County Fire Department were sent to the area of Palmdale Road approximately 34 feet west of mile marker 18 in Victorville for a report of a possible head-on collision. When officers arrived they found William Richard Bland, a 74 year-old resident of San Bernardino, was the driver and solo occupant of a blue 2008 two door Ford Focus traveling westbound on Palmdale Road. For an unknown reason the vehicle drifted into the eastbound lanes striking a white four door Toyota Corolla. The driver of the Toyota was taken by helicopter to Arrowhead Regional Medical Center for treatment of injuries. Bland was declared was declared dead at the scene at 6:56 AM, by San Bernardino County Fire Department Paramedics. The California Highway Patrol is investigating the collision. [01042012 1528 SY]

  33. Coroner case # 701200007 / 701200008 -- On 01-01-12, at 6:39 AM, Victorville City Police Department and San Bernardino County Fire received a 911 call to El Evado Road just south of Mojave regarding a solo vehicle accident. Twenty-nine year old Victorville resident Quan Campbell and 21 year old Pomona resident Amber Thompson were both pronounced dead upon emergency services arrival. The investigation revealed that the vehicle, which was driven by Campbell, was heading northbound on El Evado at a high rate of speed and failed to negotiate the curve. The vehicle left the roadway and rolled over several times. A third passenger was flown by airship to Arrowhead Regional Medical Center and treated for his injuries. The Sheriff Department's MAIT team investigated the accident. [010112 1630 TC]

  34. CORONER CASE #701108753--On 12-30-11 at 11:55 PM, a 61 year old man from Rialto was driving a dune buggy in the off-road area of Dumont Dunes when he climbed a steep embankment, went airborn and then collided with the ground. Emergency personnel responded and pronounced him dead at the scene. When next of kin has been notified, his name will be released. CHP is investigating the collision.   35. Coroner Case 06-9509DC--The San Bernardino County Sheriff's Department-Coroner Division is asking for public assistance. Back on 11/30/2006 Thomas Leroy Barton age 65, died of natural causes at San Antonio Community Hospital. He did not list anyone as his next of kin. He had also used the name of Roy Barton Thomas. He is believed to have been a transient in the Ontario area since 1998. Anyone with information as to his family is urged to contact the Coroner’s Division 909-387-2978. [120906 2025 RK]

  36. Coroner Case #06-9277CW—The Coroner’s Division of the San Bernardino County Sheriff’s Department is seeking the assistance of the media and public in locating the next of kin for Gilbert Manuel Guzman, age: 53, (DOB:09/01/1953). Guzman died of apparent natural causes, on December 1, 2006, at a friend’s home in the 4000 block of Linson Street, in the city of Adelanto.   37. Coroner Case #06-8824WB—The Coroner’s Division of the San Bernardino County Sheriff’s Department is seeking the assistance of the media and public in locating the next of kin for Enrique Valenzuela, age: 53, (DOB: 4/21/1953). Back on 11-14-06, he was found dead of apparent natural causes at his home in the 2100 block of W. Ogden Street, San Bernardino. Valenzuela is believed to have a daughter, Xochitl Valenzuela, living in Compton, California. Anyone with information is asked to call The Coroner’s Division at (909) 387-2978. [112906 2227 EM]

  38. Coroner case 06-8806 ML: Back on Tuesday, November 14, 2006, at 1:12 PM, a gardener clearing brush behind a business in the 1300 block of E. D Street saw a body near the back fence of the property and contacted the Ontario Police Department. Officers responded to the scene and confirmed the presence of a dead male adult with upper body trauma. The Ontario Police Department Homicide Detail is investigating the circumstances of the death. UPDATE (11/22/06): All attempts to identify the decedent have been unsuccessful and he has been designated John Doe #36-06. The body is described as being that of a Hispanic male, appearing to be in his late teens to twenties, 5’4”, 113 lbs, with short black hair. He had multiple tattoos, including the following: A sunburst design with a heart in the middle to the upper right chest, a mask (clown?) to the upper left arm, "Mikdalia" to the left forearm, and a skull and crossed bones to the upper right arm. One finger on his left hand had been amputated at the knuckle. Some people in the area knew him as “Marco Antonio Aguello Perez,” possibly from Chiapas.
