  1. Coroner case #700907655--- On 11/17/2009 at 7:11 PM, Angelina Bandino, a 15 year-old female resident of Hesperia, was struck by a pick-up truck while walking northbound on the shoulder of Cottonwood Avenue in the city of Hesperia. Bandino was pronounced dead by paramedics on scene at 7:20 PM. The San Bernardino Sheriff’s Department, Hesperia Station, is investigation the incident. [111809 0456]

  2. Coroner case #700907660-- On 11/17/2009, at 9:22 PM, San Bernardino Police Department officers responded to the intersection of West 40th St. and Electric Ave. in the city of San Bernardino for a report of shots fired. Donzell Tate, Jr., a 19 year old resident of San Bernardino, was found with trauma to the body. Tate was pronounced dead on-scene by paramedics at 9:25 PM. An autopsy will be conducted to determine the cause of death. San Bernardino Police Department, Homicide Division, is investigating the incident.   3. Coroner case # 700907598--On November 16,2009 at 6:08 AM a semi tractor and trailor was traveling east on Interstate 40 in the rural Ludlow area. At mile marker 78.5 the semi struck a guard rail and overturned. The driver was taken to a Barstow hospital. Passenger Tiron Jackson, a 55 year old resident of Moreno Valley was in the sleeper. Jackson was pronounced dead at the scene. The accident is under invesitgaqtion by the California Highway Patrol. [111709 1037 SY]

  4. Coroner case #700907583--- On 10/02/2009 at 3:33 PM, Deborah Valdivia, a 54 year-old resident of San Bernardino was struck by a mini van while crossing the street in the area of Foothill Ave and Pepper Ave, in the city of Rialto. Valdivia was transported to Arrowhead Regional Medical Center where she died on 11/15/2009 at 9:40 PM from her injuries. The Rialto Police Department is investigation the incident. [111609 0050 GB]

  5. Coroner case #700907581-- On 11/15/2009 at 1311, a 911 call was received reporting a traffic collision on US 95 South approximately 17 miles south of Needles. A 1988 Chevrolet pickup pulling a horse trailer was traveling northbound when it was struck by a 2009 Dodge Charger which had crossed a double yellow line in an effort to pass another car. The driver of the Chevrolet was killed in the collision. The name of the deceased is being withheld pending notification of the next of kin. The California Highway Patrol is investigating.   6. Coroner case # 700907580--On Sunday, November 15, 2009 at about 8:20 am, deputies and medical staff responded to the 12000 Block of Eaby Rd in Phelan after a family member found Phyllis Taylor, age 81, outside her residence and unresponsive. Upon arrival deputies from the Victor Valley Station, located Taylor unresponsive on her property and confirmed she was dead. The Sheriff’s Homicide Detail responded to assist and is conducting the investigation at this time. [111609 0050 GB]

  7. Coroner case # 700907556--On 11-14-09, at about 5:45 AM, hours, Lowell P. Sheehan, age: 27, from Redlands,, was driving an off-road Polaris RZR/S quad at a high rate of speed in the desert area behind the Flying J truck stop in Barstow when he went airborne over a 40 foot embankment and impacted the upper rear portion of a tractor-trailer. Paramedics responded and pronounced him dead at the scene. The Barstow Police department is investigating the incident. [111409 1200 EM]

  8. Coroner case #700907500--On 11/12/2009 at approximately 3:34 AM, A 1995 Freightliner pulling a tank trailer was traveling northbound on I-15, 6 miles north of Baker, when it was struck to the rear by a 2005 Hino box truck. The driver of the Hino was pronounced dead at the scene. The name of the deceased is being withheld pending notification of the next of kin. The California Highway Patrol is investigating.   9. Coroner case #700907494 – On November 11, 2009, at approximately 4:00 p.m., Michael Hughes, a 26 year-old resident of Joshua Tree, was riding his 2007 Yamaha motorcycle, eastbound on State Highway 62 at Samel Drive when another vehicle turned into Hughes' path. Hughes attempted to avoid the collision by swerving into the westbound lanes where he collided with another vehicle. Hughes was helmeted and was ejected from the motorcycle. Hughes was pronounced dead at the scene. The collision is being investigated by the California Highway Patrol. [11-11-09 2215 TC]

  10. Coroner case #700907479--On 11/10/09 at apprximately 8:08 PM Darlene Johnson, a 65 year old resident of Highland was walking west on the north side of Southwood Lane in Highland. At the intersection with Westwood Ln. Johnson was struck by a 2002 Chevrolet pick up turning left onto Westwood Ln from Southwood Ln. The driver called 9-1-1. Paramedics responded and Johnson was pronounced dead at the scene. San Bernardino County Sheriff's deputies from the Highland Station are investigating the accident. [111109 1103 SY]

  11. Coroner case #700907453 -- On 11-09-09, at 8:09 PM an unidentified Black male adult was a pedestrian in the roadway of westbound Palmdale Road at Vincent Road in the city of Victorville. The male was struck by a 2006 Toyota Corolla. The driver of the Toyota stopped and called 911. The pedestrian was pronounced dead at at the scene by paramedics. The decedent's name will be released following positive identification and notification of next of kin. The San Bernardino County Sheriff's Department is investigating the collision.   12. Coroner case #700907447 -- On November 9, 2009 at about 5:00pm Tsuruko Ishiguro, a 93 year old resident of Redlands, was the passenger in a 2003 Honda Odyssey that was traveling northbound on Anderson Street in Loma Linda. The Honda was approaching the intersection of Anderson Street and Academy Street and entered the left turn lane. The Honda made the left turn entering the intersection and collided with a Nissan that was traveling southbound on Anderson Street. Ishiguro was transported by paramedics to Loma Linda University Medical Center and was pronounced dead at 6:39 hours as a result of her injuries. The San Bernardino County Sheriff’s Department is investigating the collision. [111009 0918 SY]

  13. Coroner Case #700907444-- On 11/09/2009 at approximately 4:30 PM, Trevor Neiman, a twenty-five year old resident of Pinon Hills, was attacked while installing cable television at a residence in the 15200 block of San Jose Drive in Victorville. Neiman was transported to Victor Valley Community Hospital where he was pronounced dead in the emergency room at 5:06 PM. The San Bernardino Sheriff's Homicide Division is investigating this incident. [111009 0915 SY]

  14. Coroner case #700907442 – On 11/9/09 at 12:39 PM, Conrad Lewis Vega a 23 year old resident of Victorville was driving his 2002 Kawasaki ZX900f at a high rate of spped westbound on Nisqualli Road 600’ west of 4th Avenue, Victorville. He lost control and struck the north curb, left the roadway and struck a tree. Vega was wearing a helmet. This collision is also being investigated by the Victorville Police Department.   15. Coroner case #700907413 -- On 11-7-09, at 11:55 AM, 29 year-old David Arthur Padilla Jr., of El Monte, CA, was found deceased with upper body trauma under a freeway overpass in the 3800 block of Walnut Avenue in the city of Chino. Chino Police Department Homicide Division is handling the investigation. [110809 0500 TC]

  16. Coroner case #700907405 -- On 11-7-09, at 4:15 PM, Andrew Ryan McKenzie, a 26 year-old resident of Costa Mesa, was the backseat passenger in an off-road vehicle on the Calico dry lake bed in Yermo approximately 3 miles south of Calico Ghost Town in open desert area. The driver of the vehicle lost control and rolled over several times at which time McKenzie was ejected and crushed. Emergency personnel arrived on-scene at 4:40 PM and confirmed McKenzie deceased.   17. Coroner case #700907398, 700907399 and 700907400--On 11-7-09 at about 8:30 AM, a vintage twin rotor helicopter crashed in the open desert northwest of Palmdale Road and Aster Road in Adelanto. Emergency personnel responded and found the wreckage on fire which was extinguished. Afterwards the remains of 3 male adults were found burned beyond recognition. When their identities have been determined by dental records and next of kin notified, their names will be released. The San Bernardino County Sheriffs Dept and F.A.A. are investigating the incident.   18. Coroner case #700907390 -- On 11-06-09, at 3:36 PM, Victorville City Police Department received a 911 call of a domestic violence with a possible stabbing in the 16500 block of Majela Avenue. Deputies responded and rescued a female, who sustained upper body trauma, outside of the residence. A male subject, later identified as Simmie Jones, a 61 year-old resident, barricaded himself inside the residence. The San Bernardino County Sheriff SWAT team arrived and assumed negotiations. After continued unsuccessful attempts to make contact with Jones and have him exit the residence the SWAT team made entry into the residence at 7:55 PM finding Jones dead from a self-inflicted gunshot wound. An autopsy will be performed to determine the exact cause of death. The Victorville Police Department is investigating the incident. [110709 0200 TC]

  19. Sheriff Department Missing Person Case #010903389: Back in February 1993 Doungboupa “Dawn” Sihakhom, a 17 year old resident of Ontario, California, ran away from home. After losing contact with Dawn, the family believed that she may have met with foul play, and contacted their local law enforcement agency. However, no missing person report was accepted by the police at that time On July 13, 2009, San Bernardino County Sheriff Volunteer Becky Castillo (Coroner Division) was contacted by a family member. After arranging for Dawn to be reported missing, Volunteer Castillo assisted the family in submitting reference DNA samples, consisting of q-tips swabbed on the inside of a family member’s mouth, and submitting these to the California Department of Justice, Missing Persons DNA Program. Volunteer Castillo then began searching records of reported unidentified person, living and dead. Castillo located a case of an unidentified young female who had been found dead in Sierra Madre, Los Angeles County, on February 7, 1993, the victim of a homicide. DNA samples had been submitted by the Los Angeles County Coroner Department for Jane Doe #7, and a profile was available. Castillo requested that the two profiles be compared, as soon as the missing person’s profile was completed, a process that typically takes approximately four months. On Monday, October 26, 2009, the Department of Justice confirmed that Jane Doe #7 was Dawn Sihakhom. Dawn’s death is under the investigation of the Los Angeles County Sheriff Department.   20. Coroner Case #700907374--On 11/06/09 at 6:38 AM fire personnel from the San Bernardino Fire Department responded to the 1600 Block of Lugo Ave. for a structure fire. When they arrived, fire department paramedics found Leta Carol Lindsey, a 65 year-old resident of San Bernardino unresponsive inside of her apartment. Paramedics pronounced Lindsay dead at the scene. The incident is being investigated by the San Bernardino Police Department and the San Bernardino City Fire Department. An autopsy will be conducted next week to determine the exact cause of death.   21. Coroner Case #700907290—On 11/02/2009 at approximately 11:25 PM, San Bernardino Sheriff’s Department deputies responded to a 911 call for a male laying on the side of the road in the 8100 block of Palomar Ave in Yucca Valley. Douglas Justin James Willison, a 56 year old resident of Yucca Valley had been riding his bicycle and was rear-ended by a pick-up truck. He was pronounced dead at the scene by paramedics at 11:33 PM. The San Bernardino Sheriff’s Department is investigating the collision. [110309 0940 SY]

  22. Coroner case #700907278-- On Monday, 11/02/2009 at approximately 12:00 PM, sixty year old Celebrada Lomato-Sexton was found shot to death in a cabin at Camp Cedar Falls Campground in Angeles Oaks. Paramedics arrived on scene and pronounced Lomato-Sexton dead at 1222 hours. Lomato-Sexton was a resident/employee at the campground. The San Bernardino County Sheriff's Homicide Detail is investigating. [110209 2136 GB]

  23. Coroner case #700907236 -- On 11-01-09, at 1:42 AM a 47 year-old male from Mira Loma was walking northbound in the southbound #2 lane of Interstate 215 just south of the Devore overpass in Devore. The pedestrain was struck by a 2005 Ford Explorer. The driver of the Explorer stopped and called 911. The pedestrian was pronounced dead at 2:00 AM. When family of the decedent is contacted and notified his name will be released to the public.   24. Coroner Case #700907241-- On 11/01/2009, at 12:33 AM, San Bernardino County Sheriff Department deputies responded to a residence located in the 35000 block of Ave C, in the city of Yucaipa for a report of an assault. Bianca Becerra, a 16 year old resident of Yucaipa, was found with trauma to the body. Becerra was transported to Loma Linda Medical Center where she later died at 4:00 AM from her injuries. An autopsy will be conducted to determine the cause of death. San Bernardino County Sheriff Department, Homicide Division, is investigating the incident. [110109 0600 TC]

    1. Coroner case #700705335 - Back on July 11, 2007, Kelly Gene Saunders, a 46 year old resident of San Bernardino, died as the result of a house fire in the 200 block of East 17th Street, in San Bernardino. The Coroner's Division has been unsuccessful in locating his family and is seeking the public's assistance in helping us to find them. If anyone knows of any relatives of Saunders, please contact the Coroner's Division at 909-387-2978 [071707 0358 CN] Family was located and notified of the death. No further assistance is needed.     2. Coroner Case 06-9509DC--The San Bernardino County Sheriff's Department-Coroner Division is asking for public assistance. Back on 11/30/2006 Thomas Leroy Barton age 65, died of natural causes at San Antonio Community Hospital. He did not list anyone as his next of kin. He had also used the name of Roy Barton Thomas. He is believed to have been a transient in the Ontario area since 1998. Anyone with information as to his family is urged to contact the Coroner’s Division 909-387-2978. [120906 2025 RK]

    3. Coroner Case #06-9277CW—The Coroner’s Division of the San Bernardino County Sheriff’s Department is seeking the assistance of the media and public in locating the next of kin for Gilbert Manuel Guzman, age: 53, (DOB:09/01/1953). Guzman died of apparent natural causes, on December 1, 2006, at a friend’s home in the 4000 block of Linson Street, in the city of Adelanto. Guzman is believed to have lived previously in the Los Angeles area. Anyone with information is asked to call The Coroner’s Division at (909) 387-2978. [120206 2255 EM]

    4. Coroner Case #06-8824WB—The Coroner’s Division of the San Bernardino County Sheriff’s Department is seeking the assistance of the media and public in locating the next of kin for Enrique Valenzuela, age: 53, (DOB: 4/21/1953). Back on 11-14-06, he was found dead of apparent natural causes at his home in the 2100 block of W. Ogden Street, San Bernardino. Valenzuela is believed to have a daughter, Xochitl Valenzuela, living in Compton, California. Anyone with information is asked to call The Coroner’s Division at (909) 387-2978. [112906 2227 EM]

    5. Coroner case 06-8806 ML: Back on Tuesday, November 14, 2006, at 1:12 PM, a gardener clearing brush behind a business in the 1300 block of E. D Street saw a body near the back fence of the property and contacted the Ontario Police Department. Officers responded to the scene and confirmed the presence of a dead male adult with upper body trauma. The Ontario Police Department Homicide Detail is investigating the circumstances of the death. UPDATE (11/22/06): All attempts to identify the decedent have been unsuccessful and he has been designated John Doe #36-06. The body is described as being that of a Hispanic male, appearing to be in his late teens to twenties, 5’4”, 113 lbs, with short black hair. He had multiple tattoos, including the following: A sunburst design with a heart in the middle to the upper right chest, a mask (clown?) to the upper left arm, "Mikdalia" to the left forearm, and a skull and crossed bones to the upper right arm. One finger on his left hand had been amputated at the knuckle. Some people in the area knew him as “Marco Antonio Aguello Perez,” possibly from Chiapas.
