Immunizations
The Immunization Program provides comprehensive immunization services to reduce the incidence of specific diseases known
to be preventable by immunization. Immunization Program services include

Children's Physicals and Immunizations

Immunizations for babies should be part of a well-baby examination. Referrals for low-cost well-baby examinations are available through the Child and Family Health Services at 1-800-722-3777. The Clinic Operations Section also offers children’s immunizations by appointment at clinics countywide. An appointment can be made by calling 1-800-722-4777.

Traveler's Vaccines

The Clinic Operations Section offers travel vaccines at cost at limited clinic sites. You can find the locations and schedules for Travel Clinics at the following link: Travel Clinics or by calling 1-800-722-4777.

Immunization Registry

The Immunization Program maintains an immunization registry under the provisions of State law. The purpose of the registry is to assure accurate and complete immunization records are available to all health care providers serving children with immunization services. Information for health care providers interested in participating is available from the Immunization Registry Help Desk at 1-800-722-4794.

Records
If you or your child received medical care through

the medical records by contacting the appropriate program. Immunization
records on children can
be requested by contacting the Clinic Operations Section at
1-800-722-4777.  If your child’s immunizations were given as part of a well-baby exam or child health physical, contact the Child and Family Health Services at 1-800-722-3777 or 909-388-0400. You will be required to sign an authorization form to allow the release of medical records. This may be subject to a fee.

The Clinic Operations Section provides year round pediatric immunization clinics, adult and travel immunizations at selected sites throughout the county.

Immunizations For Kindergarten and Childcare

Immunizations prevent diseases from occurring in children and being transmitted to others. California law requires children entering kindergarten and child care to be appropriately immunized to protect themselves and others. Children and school records are screened to assure that children receive appropriate immunizations. Information on kindergarten and other school entry requirements is available from the Immunization Program at 1-800-722-4794.

Immunization Provider Services

to prevent diseases from occurring in all ages.  Immunization program staff works with immunization providers in public and private sectors to educate providers on new trends and requirements as well as general techniques to improve immunization practices in San Bernardino County.  We provide personalized training on the immunization schedule, vaccine storage and handling and the CAIR immunization registry all in an effort to increase vaccine coverage rates.  If you are a provider and would like to have a personalized consultation for your practice, call 1-800-722-4794 and ask to speak with one of our health educators. 

Adolescent Immunizations

Are you 11 through 18 years of age (prior to 19th birthday)? Vaccines for adolescents include meningococcal, human papillomavirus, Tdap (tetanus, diphtheria, and pertussis), MMR (measles, mumps, rubella), and varicella (chickenpox) to ensure the adolescent is protected. An appointment can be made by calling 1-800-722-4777.
