Blue Shield PPO Bronze Plan

Open Enrollment: May 19-30, 2014

The County of San Bernardino is offering employees who meet the following criteria an opportunity to enroll in the Blue Shield PPO Bronze Plan:

  • ✔ Work an average of 30 hours or more a week during our look back period
    (May 4, 2013 - May 2, 2014) and

  • ✔ Not eligible for other County-sponsored medical coverage

If you are eligible to enroll in the Bronze Plan, you will receive a letter with information about the Plan and steps you need to take to enroll in or decline the Plan. View the letter.

Note: If you are eligible for the Bronze Plan, you must enroll in or decline medical coverage by May 30, 2014.

Frequently Asked Questions

  1. Can I enroll my dependents in the plan?
    Yes. You have the option to enroll your dependents in medical coverage under the Bronze PPO plan.

  2. When will my coverage take effect?
    Coverage is effective July 26, 2014, for those who enroll during Open Enrollment.

  3. When will I receive my subscriber ID cards?
    Those who enroll in the Bronze plan will have their ID cards mailed to their home within two weeks of the effective date of coverage. Information on how to request an ID card can be found here.

  4. How much will the plan cost and what are the benefits?
    The plan costs and a summary of the plan benefits can be found here. Please be sure to carefully review the plan benefits and costs before enrolling.
