The Child Health & Disability Prevention (CHDP) Program provides free preventive health examinations to children throughout San Bernardino County. There are over 180 health assessors in San Bernardino County who are approved to provide CHDP examinations. Children who go in for a CHDP
exam receive many benefits, including
screenings for vision, dental and
hearing, nutrition assessments, tests for diabetes and anemia and much more.
The CHDP Program has become a "gateway" for children under 19 years of age to become enrolled in Medi-Cal and Healthy Families. The CHDP Gateway Program automatically enrolls eligible children into no cost, fee-for-service Medi-cal for the remainder of the month of the CHDP exam, and the following month. The CHDP provider office will provide the family with information about the CHDP Gateway process and how to obtain help in applying for these programs. Even if the child is not eligible for Medi-Cal or Healthy Families, he/she can still receive CHDP services.
Target Population
Non-Medi-Cal Eligible children and youth: Ages birth to 18 years.
Fee for service Medi-Cal children and youth from birth through 20 years of age.
Children attending
Head Start and State Preschool programs.
The Program:
Assist in obtaining health assessment and dental services for the target population.
Offers a full range of services which include a health history; physical examination; dental assessment; nutritional assessment; developmental assessment; immunizations; vision and hearing testing; some laboratory tests (e.g. tuberculin, lead, hemoglobin/
hematrocrit); health education (information on the harmful effects of the use of tobacco products and exposure to second hand smoke; and anticipatory guidance).
