How to Receive and/or Submit a Proposal

1. All applicants must complete an application packet, generally available in May of each year. The application may be typed, computer printed, or neatly and legibly handwritten in ink. Applications that are not legible will be denied.

2. Submit one (1) original copy of the application prior to the submission deadline and maintain one copy on file at your agency. If the project involves another organization, please provide letter(s) of intent to collaborate.

3. The application package is available from the DSHN by e-mail. To receive a copy or ask questions pertaining to the process, the budget, or allowable expenses, contact:

Mini-grant Project Previously Funded by Desert Sierra Health Network

Preschool Services Department of San Bernardino County Family And Community Educating Together Program (FACET)
To educate parents of enrolled Head Start/State Preschool children on the need to increase daily consumption of fruits and vegetables and daily physical activity among children between ages three to five in San Bernardino County. This will include training a parent at each site to be peer trainers to conduct 4 weekly nutrition education/pa promotion classes with food demonstrations for other site parents.
