How to Receive and/or Submit a Proposal
1. All applicants must complete an application packet,
generally available in May of each year. The application may be typed, computer printed, or neatly and legibly handwritten in ink. Applications that are not legible will be denied.
2. Submit one (1) original copy of the application prior to the submission deadline and maintain one copy on file at your agency. If the project involves another organization, please provide letter(s) of intent to collaborate.
3. The application package is available from the DSHN by e-mail. To receive a copy or ask questions pertaining to the process, the budget, or allowable expenses, contact:
