Sierra Health Network-Mini-grants
Program Overview
The Desert Sierra Health Network (DSHN), provides coordination and support for community nutrition education and physical activity promotion to achieve the goals of the Network for a Healthy California in Inyo, Riverside, and San Bernardino Counties. San Bernardino County Department of Public Health Nutrition Program (Fiscal Agent) administers the mini-grants on behalf of DSHN.
The goal of the DSHN is to: Increase access to nutritious food and physical activity in order to reduce health disparities and promote lifelong health. An objective of the DSHN is to: Empower Food Stamp Nutrition Education (FSNE) eligible families to advocate for community and school nutrition, and physical activity and wellness initiatives.
Mini-grant Program Goal
The DSHN is pleased to announce the availability of funds of $500 to $4999 per grant, through the U.S.D.A. Food Stamp* Nutrition Education Program, for short-term, creative projects reaching low-income populations that work toward obesity reduction by:

Training the target population to make healthful food decisions through nutrition education using an empowerment approach which mobilizes them to pursue improvements in their community to access healthier foods, specifically fruits and vegetables, and physical activity opportunities. |

Promoting/encouraging and educating on the need to increase daily consumption of fruits and vegetables and daily physical activity. |

Who can apply?
Any non-profit organization or agency may apply including community organizations, schools, and governmental agencies; religious, social, and civic organizations.

Target Population
The specific target audience for this program must be residents of one of the following counties: Inyo, Riverside, or San Bernardino who are recipients of, or eligible to receive Food Stamps, and similar households living at or below 185% of the federal poverty level. This may include service populations of other programs, such as Head Start, Women, Infants and Children Services (WIC); Medi-Cal; or school districts or schools where 50% or more of the students are eligible for the free or reduced lunch program. Refer to Income Eligibility Guidelines on the next page.
Targeting data is required and must be referenced in the proposal and include the source of the data and location for the intervention site. Examples are:
