IMMUNIZATIONS
Pediatric immunization clinics, adult immunization, traveler's immunizations.
The Immunizations Program provides comprehensive immunization services to reduce the incidence of specific diseases that are known to be preventable by immunization.

The program services include year round pediatric immunization clinics, seasonal adult immunization services for influenza and pneumococcal pneumonia, travel immunizations, and activities for prevention of Hepatitis B during the perinatal period.

Immunizations For Kindergarten
and 7th Grade
Immunizations prevent diseases from occurring in children and being transmitted to others. California law requires children entering kindergarten and 7th grade to be appropriately immunized to protect themselves and others. Information on kindergarten and other school entry requirements is available from the Immunization Program at 1-800-722-4794.

Children's Immunizations
Immunizations for babies should be part of a well-baby examination. Referrals for low-cost well baby examination are available through the Child Health Disability Prevention Program at 1-800-722-4542. The Department also provides children’s immunizations at Immunization-Only walk-in clinics countywide. The clinic schedule is available by calling 1-800-722-4794 or checking the immunizations clinic schedules.

Immunization Registry
The Department maintains an immunization registry under the provisions of State law. The purpose of the registry is to assure accurate and complete immunization records are available to all health care providers serving children with immunizations services. Information for health care providers interested in participating is available from the Immunization Registry Help Desk at (909) 386 - 8332.

Medical Records
If you or your child received medical care through the health department, you can obtain a copy of the medical records by contacting the appropriate program. Immunization records on children can be requested by contacting the Immunization Program at (909) 383-3070. If your child’s immunizations were given as part of a well baby exam or child health physical, contact the Child and Adolescent Health Program at 1-800-722-3777 or (909)388-0400. You will be required to sign an authorization form to allow the Department to release the medical records and may be subject to a fee of $7. |
