Agency Description:

Provides service in the areas of physical health care, substance abuse treatment, mental health improvement, homeless services and prevention education. It is a member of the Central Valley Health Network, a nonprofit corporation, which provides services to low-income and medically underserved persons in San Bernardino County.

Program: Project Home Again

Program Description: Project Home Again is a HUD funded Supportive Services Only program. Using a client center approach. Project Home Again staff helps individuals and families find and remove the root cause of their homeless condition through: Counseling, education, guidance and troubleshooting.

Funding Sources: Supportive Housing Program (HUD-SHP)
