How Homeless Management Information System Works

 

SBC CoC HMIS Software

HMIS is a software application system designed to record and store client-level information on the characteristics and service needs of homeless persons.  Specifically, HMIS is used to produce unduplicated count of homeless persons, understand patterns of service use, and measure the effectiveness of homeless programs.  HUD further encouraged local communities to determine their own best way to implement such a system.

SBC CoC HMIS is a comprehensive case management system that allows the agency users to use the collected information to make informed program decisions. Additionally, the SBC CoC HMIS includes a focus on Outcomes Management, which is intended to provide value by allowing the user to set and measure, client and program milestones, and target achievements.

HMIS Implementation

HMIS Implementation requires technical and programmatic assistance. The SBC CoC HMIS Team is charged with the implementation of the outcome management component and program setup in the SBC CoC HMIS application. This includes the agency outcomes for milestones, performance targets as it related to the contracted services with HUD and/or other funders.

The SBC CoC HMIS staff also in charged of system security setup in the SBC CoC HMIS application, software installation, data input and report training, user support and data analysis for SBC CoC HMIS participating agencies.

As of October 2008, there are 24 agencies with 31 programs in SBC CoC HMIS. The team has provided outcomes and user training to an estimated 135 users.

HMIS Implementation Process

Step 1: HMIS Orientation.  HMIS orientation is conducted once a month to homeless service providers to provide an overview of HMIS, HUD mandate, as well as benefits and requirements for participation.
Step 2: HMIS Technical Assessment and Installation.  After the orientation, agencies interested in implementing HMIS will sign the Memorandum of Agreement and will be provided with a copy of HMIS Policies and Procedures including materials for posting and other forms.  HMIS staff visits the agency for site inspection and installation of HMIS equipment computer system provided to participating agencies under HUD grant funding. These equipments are on long term loan for the purpose of collecting client data information and tracking services.
Step 3: Outcomes Management Training. Establishing Milestones and Performance Targets.  This training is conducted with the program manager or case manager to identify milestones and performance targets.   This includes services provided to homeless clients with a detailed description of how those services assist participants to achieve the milestones and ultimately complete the program.  This Milestone/Performance is customized for each participating agency.
Setup 4: User Training.  Users Training is conducted twice a month by a trainer from Adsystech, Inc..  All HMIS users are required to attend this training before utilizing HMIS. After the training, participants are given Certificate of Completion, user name and password.
Step 5: Go Live! and Technical Support. With the completion of Steps one to four, the agency is now ready to “Go Live” or start using the system.   Technical support is provided by calling 909-723-1523 or email hmishelpdesk@capsbc.sbcounty.gov
